Identification of microbes from blood cultures using MALDI-TOF mass spectrometry

Abstract:

Infekce krevního řečiště (IKŘ) se vyznačují vysokou morbiditou a mortalitou. Nejčastějšími původci těchto stavů jsou v současnosti gramnegativní bakterie v čele s Escherichia coli a dalšími zástupci z čeledi Enterobacteriaceae, následované grampozitivními koky jako Staphylococcus aureus, koaguláza negativními stafylokoky nebo enterokoky. Do popředí se dostávají také kvasinky rodu Candida zodpovědné za velkou část nozokomiálních IKŘ. Klíčem k úspěšné terapii je mimo jiné rychlá identifikace původce a jeho citlivosti k antimikrobiálním látkám. Zlatým standardem diagnostiky IKŘ je hemokultivace v automatizovaných přístrojích s využitím hemokultivačních lahviček. Následná biochemická identifikace patogenu může trvat i několik dní a snahou je její zkrácení. V současné době se do klinických laboratoří rozšiřuje hmotnostní spektrometrie MALDI-TOF, která představuje spolehlivou, rychlou a poměrně levnou metodu identifikace, založenou na analýze proteinového spektra zkoumaného mikroorganismu. Pro přímou identifikaci z hemokultur je nutná příprava vzorku spočívající v oddělení mikroorganismu od krve a kultivačního média a extrakce mikrobiálních proteinů. Cílem této práce bylo otestovat metody přípravy vzorků z hemokultur a vybrat metodu, která by byla vhodná pro získání orientačního výsledku v běžném provozu Mikrobiologického ústavu Lékařské fakulty Masarykovy univerzity a Fakultní nemocnice u sv. Anny.

Abstract:

Bloodstream infections (BSI) are characterized by high morbidity and mortality. The most common agents of these conditions are currently gram-negative bacteria, lead by Escherichia coli and other representatives of the Enterobacteriaceae family, followed by gram-positive cocci such as Staphylococcus aureus, coagulase-negative staphylococci or enterococci. Moreover, Candida yeasts are responsible for a big part of nosocomial BSI. The key to successful therapy is, among other things, rapid identification of the agent and its susceptibility to antimicrobials. The gold standard of BSI diagnostics is hemoculture in automated devices using blood culture vials. Subsequent biochemical identification of the pathogen may take several days, and effort is being made to shorten it. MALDI-TOF mass spectrometry is a reliable, fast and relatively inexpensive method of identification based on the analysis of the protein spectrum of the microorganism. At present, its usage expands in clinical laboratories. For direct identification from blood cultures, preparation of a sample consisting of separation of microorganism from blood and culture medium and extraction of microbial proteins is necessary. Purpose of this work is to test the methods of preparation of samples from blood cultures and to select a method that could be used to obtain an orientation result in the routine operation of the Institute of Microbiology of the Faculty of Medicine of Masaryk University and St. Anne's University Hospital.
