Clinical and Functional Investigations of Lower Limbs Venous System Disease and Their Correlation

Abstract:

V souboru 50 pacientů s projevy CVD na obou DKK, kteří v období let 2004-2006 navštívili flebologickou ambulanci, bylo provedeno hodnocení klinického nálezu dle CEAP klasifikace a provedeno vyšetření žilního systému dolních končetin digitální fotopletyzmografií a ultrazvukem. Porovnání výsledků bylo provedeno na 100 dolních končetinách s klinickými projevy C1-C6 dle CEAP. V souboru bylo 18 mužů a 32 žen. Poměr ženy:muži byl 1,8:1. Průměrný věk pacientů v souboru byl 44,68 ± 10,72 roku. Nejčastěji se onemocnění žilního systému objevilo ve skupině pacientů mezi 40-49 rokem věku. Průměrná doba trvání CVD byla u pacientů souboru 8,27 ± 8,72 roku. CEAP klasifikace Třída C: Na základě hodnocení klinického obrazu dle CEAP klasifikace do třídy C1 bylo zařazeno 27 pacientů, do třídy C2 39 pacientů, do třídy C3 a do třídy C4 po 14 pacientech a ve třídě C5,6 bylo 6 pacientů. Třída E: Na základě anamnestických údajů a i dle výsledků ultrazvukového vyšetření byla u pacientů určena etiologie chronické žilní nemoci. U 92 pacientů byla zjištěna primární etiologie varixů, 8 pacientů mělo sekundární varixy, u 1 pacienta se sekundárními varixy byla ultrazvukem prokázána 50% trombotická okluze ve VP. Třída A: Na základě výsledků ultrazvukového vyšetření a klinického vyšetření žil na dolních končetinách bylo zjištěno postižení povrchového žilního systému ve 45 případech, postižení hlubokého žilního systému ve 23 případech, postižení perforátorů na 3 končetinách a bez postižení či prokazatelné anatomické anomálie byla končetina u 42 pacientů. Třída P: V souboru pacientů byl v 57 případech zjištěn reflux v povrchovém nebo hlubokém žilním systému, u 1 pacienta byla v hlubokém žilním systému dolní končetiny zjištěna 50% obstrukce. U 42 pacientů nebylo zjištěno postižení žilního systému. D-PPG vyšetření U 46 pacientů byla zjištěna normální hodnota žilního plnícího tlaku, porucha žilně svalové pumpy I.stupně byla u 15 pacientů, II.stupně u 28 pacientů a porucha III.stupně byla zjištěna u 11 pacientů. Dopplerovská ultrasonografie U pacientů bylo provedeno vyšetření žilního systému DKK ultrazvukem. U 42 pacientů nebylo zjištěno postižení žilního systému, u 45 pacientů bylo zjištěno postižení povrchového žilního systému a u 27 pacientů bylo zjištěno postižení hlubokého žilního systému, přičemž u 1 pacienta byla zjištěna 50% trombotická okluze ve VP. Vzhledem k tomu, že se jednalo o postižení 1 pacienta a vzhledem k odlišnosti typu postižení, nebyl tento pacient zahrnut do dalších výpočtů a srovnání. U 13 pacientů bylo zjištěno postižení povrchového i hlubokého žilního systému DKK. Vzájemná korelace vyšetření Výsledná hodnota kappa koeficientu = 0,344 při porovnání výsledků klasifikace projevů CVD v třídě C a výsledků D-PPG vyšetření odpovídá slabé míře shody. Lze říci, že výsledky D-PPG vyšetření nejsou závislé na klinickém obraze změn u CVD a s klasifikací CEAP prakticky nekorelují. Hodnota kappa koeficientu 0,522 při porovnání výsledků vyšetření podle třídy C z CEAP klasifikace s výsledky ultrazvukového vyšetření odpovídá průměrné shodě. Porovnání výsledků vyšetření D-PPG s výsledky ultrazvukového vyšetření s hodnotou kappa koeficientu 0,552 odpovídá také průměrné míře shody. Hodnota kappa koeficientu je v při těchto srovnáních již výrazně vyšší,ale je nutno si uvědomit, že stále existují případy, kdy se výsledky vyšetření D-PPG a ultrazvukem v závažnosti postižení žilního systému neshodují. Koeficienty kappa při vzájemném srovnání výsledků jednotlivých vyšetření prokázaly jen nízkou či střední míru shody výsledků vyšetření. Stejně tak i stanovení interva

lu spolehlivosti, senzitivity, specificity a celkové přesnosti výsledků vyšetření D-PPG a CEAP klasifikace ve vztahu k DUS, neprokázaly dostatečnou spolehlivost těchto vyšetření, které nelze doporučit jako vyšetření první volby při diagnostice onemocnění žilního systému dolních končetin a mohou sloužit pouze jako vyšetření doplňková.

Abstract:

In the group of 50 patients with CVD symptoms at both lower limbs, who visited the ambulance of phlebology in the years 2004-2006, a clinical finding evaluation according to CEAP classification was performed and there was carried out the investigation of the lower limbs venous system via digital photoplethysmography and ultrasound detection. Results comparison was performed according to CEAP at 100 lower limbs with clinical symptoms C1-C6. The group consisted of 18 men and 32 women. Women and men ration was 1,8:1. Patients' average age in the group was 44.68 ± 10.72 year. Venous system disease arose in a group of patients between the age of 40 and 49. The average time of CVD duration at groups’ patients was 8.27 ± 8.72 year. CEAP classification Class C: Based on the clinical picture evaluation according to CEAP classification, 27 patients were included in the class C1, 39 patients were included in the class C2, 14 patients into classes C3 and C4 each and 6 patients into classes C5 and C6 each. Class E: Based on anamnestic data and also according to ultrasound examination results, chronic venous disease aetiology was determined at patients. Varicose veins primary aetiology was determined at 92 patients, 8 patients had secondary varicose veins, 50% thrombotic occlusion in VP was proved via ultrasound examination at 1 patient. Class A: Based on ultrasound examination and clinical investigation of lower limbs veins, superficial venous system infliction was determined in 45 cases, deep venous system infliction was determined in 23 cases, perforators’ infliction in 3 limbs and at 42 patients, the limb was without infliction or evincible anatomic anomaly. Class P: In the group of patients, there was reflux in lower limb’s superficial or deep venous system determined in 57 cases, 50% obstruction in lower limb’s deep venous system was determined at 1 patient. Venous system infliction was not found at 42 patients. D-PPG investigation Normal value of venous filling time was determined at 46 patients, venous muscular pump disorder of 1st grade at 15 patients, of 2nd grade at 28 patients and 3rd grade disorder was determined at 11 patients. Doppler ultrasonography Patients’ lower limbs venous system investigation was performed via ultrasound detection. At 42 patients, no infliction of the superficial venous system was determined, superficial venous system infliction was determined at 45 patients and deep venous system infliction was determined at 27 patients, while 50% thrombotic occlusion in VP was proved at 1 patient. Regarding the fact that 1 patient was inflicted and regarding the diversity of the infliction type, this patient was not included into further calculations and comparisons. Both superficial and deep infliction of the venous system of lower limbs was determined at 13 patients Mutual correlation of the investigation Resultant kappa coefficient value 0.344 corresponds with low conformity measure when comparing CVD symptoms classification results in the class C and D-PPG results. It might be said that D-PPG investigation results do not depend on CVD changes clinical picture and does not practically correlate with CEAP classification. Kappa coefficient value 0.522 corresponds with and average conformity when comparing investigation results according to C class of CEAP classification and ultrasound examination results. Comparison of D-PPG investigation results and the ultrasound examination results with kappa coefficient value 0.552 also corresponds with the average conformity measure. Kappa coefficient value is already significantly higher in these comparisons, however, it is necessary to be aware that there are still cases, in which D-PPG and ultrasound examinations do not conform in venous system infliction’s seriousness. rall accuracy of D-PPG and CEAP classification investigation results in relation to DUS, did not proved sufficient reliability of these investigations, which can not be recommended as first choice examinations at the diagnosis of lower limbs venous system diseases and they can serve as supplementary examinations only.
