Molecular targeted therapy in chronic myeloid leukaemia

Abstract:

Současný výzkum chronické myeloidní leukémie (CML) řeší dvě základní témata: (1) vymýtit CML kmenové buňky pomocí nových efektivních molekul či jejich kombinací a (2) vyvinout spolehlivou metodu pro predikci léčebné odpovědi. Předkládaná práce se zaměřuje na obě tyto témata, jejichž cílem je redukce v počtu CML pacientů rezistentních na léčbu. Tato dizertační práce je složena ze dvou experimentálních částí. První část je nazvána "MEK inhibitor PD184352 zvyšuje apoptózu vyvolanou BMS-214662 v CD34+ CML kmenových/progenitorových buňkách" a druhá část je nazvána "BCR-ABL aktivita měřená v CD34+ buňkách CML pacientů pomocí IC50imatinib, p-CrkL/CrkL poměru a p-CrkL poměru nepredikuje léčebnou odpověď". (1) Cytotoxický farnesyl transferázový inhibitor BMS-214662 indukuje mitochondriální apoptózu v primitivních CD34+ CML kmenových/progenitorových buňkách. V první části studie jsme pomocí MEK inhibitoru PD184352, který cílí na BCR-ABL downstreamovou dráhu ERK, zvýšili apoptotický účinek BMS-214662 v CD34+ CML kmenových/progenitorových buňkách. PD184352 zvýšilo apoptotický účinek BMS-214662 u buněčné linie K562 i v primárních CD34+ CML buňkách. V kombinované léčbě jsme zaznamenali inhibici fosforylace ERK, zvýšení hladiny Annexinu-V, aktivaci kaspáz-3, -8 a -9 a zvýšení poškození mitochondrií, spojené s poklesem hladiny anti-apoptotického proteinu MCL-1. Inhibice funkce K-RAS pomocí dominantně negativních mutantů vedlo k buněčné smrti CML buněk a tento proces byl umocněn přidáním BMS-214662 a PD184352. Naše výsledky naznačují, že kombinace MEK inhibitoru a BMS-214662 zvyšuje schopnost selektivně cílit na CML kmenové/progenitorové buňky, které nereagují na léčbu tyrosin kinásovými inhibitory a jsou pravděpodobně zodpovědné za persistenci a relaps onemocnění. (2) Současný výzkum odhalil, že úroveň fosforylace proteinu CrkL (p-CrkL) může sloužit jako prognostický marker léčebné odpovědi na imatinib u CML pacientů. Ve druhé části studie jsme stanovovali aktivitu BCR-ABL kinázy pomocí měření hladiny p-CrkL a CrkL, za účelem predikce léčebné odpovědi u nově diagnostikovaných CML pacientů. Před zahájením podávání imatinibu jsme pacientům odebrali CD34+ buňky a tyto jsme in vitro ovlivňovali imatinibem. Redukce hladiny p-CrkL a CrkL byla následně měřena pomocí průtokové cytometrie. Získané data byla zpracována 3 nezávislými postupy: stanovení a) IC50imatinib, b) p-CrkL/CrkL poměru, and c) p-CrkL poměru. Výsledky jsme korelovali s klinickou odpovědí pacientů po 3 a 6 měsících užívání imatinibu. Naše výsledky naznačují, že stanovení aktivity BCR-ABL v CD34+ buňkách pomocí průtokové cytometrie nepredikuje léčebnou odpověď pacientů.

Abstract:

The main issues of today's chronic myeloid leukaemia (CML) research are (1) to eradicate CML stem cells by novel effective molecules either used alone or in combination with other substances and (2) to improve methods for treatment response prediction. This work is focused on both topics, which should together contribute to the gradual reduction in CML patients resistant to therapy. PhD thesis consists of two experimental parts. Part 1 is titled "The MEK inhibitor PD184352 enhances BMS-214662-induced apoptosis in CD34+ CML stem/progenitor cells" and part 2 is titled "BCR-ABL activity measured by the IC50imatinib, p-CrkL/CrkL ratio, or p-CrkL ratio in CD34+ cells of CML patients does not predict treatment response". (1) The cytotoxic farnesyl transferase inhibitor BMS-214662 has been shown to potently induce mitochondrial apoptosis in primitive CD34+ CML stem/progenitor cells. In the first part of the study, to enhance the BMS-214662 apoptotic effect, we further targeted the extracellular signal-regulated kinase (ERK) pathway, downstream of BCR–ABL, by treating CD34+ CML stem/progenitor cells with a highly selective adenosine triphosphate (ATP) non-competitive MEK inhibitor, PD184352. PD184352 increased the apoptotic effect of BMS-214662 in a CML blast crisis cell line, K562, and in primary chronic phase CD34+ CML cells. Compared with BMS-214662, after combination treatment we observed inhibition of ERK phosphorylation, increased Annexin-V levels, caspase-3, -8 and -9 activation and potentiated mitochondrial damage, associated with decreased levels of anti-apoptotic BCL-2 family protein MCL-1. Inhibition of K-RAS function by a dominant-negative mutant resulted in CML cell death and this process was further enhanced by the addition of BMS-214662 and PD184352. Together, these findings suggest that the addition of a MEK inhibitor improves the ability of BMS-214662 to selectively target CML stem/progenitor cells, notoriously insensitive to tyrosine kinase inhibitor treatment and presumed to be responsible for the persistence and relapse of the disease. (2) Recent studies have revealed that phosphorylated CrkL protein can act as a prognostic marker for imatinib treatment response of CML patients. In the second part of the study, we assessed BCR-ABL kinase activity by measuring the levels of p-CrkL and CrkL in order to predict the clinical outcome of newly diagnosed CML patients. CD34+ cells from these patients were collected before the start of imatinib therapy and treated in vitro with imatinib. The reduction of p-CrkL and CrkL protein levels was then measured by flow cytometry. The data were processed using three independent approaches: an assessment of a) IC50imatinib, b) p-CrkL/CrkL ratio, and c) p-CrkL ratio. The results were subsequently correlated with the clinical response of patients after 3 and 6 months of imatinib therapy. Our data suggest that flow cytometric assessment of BCR-ABL kinase activity in CD34+ cells does not have the predictive power of a clinical outcome.
