Effect of continuous glucose monitoring for diabetes control in pediatric patients

Abstract:

Téma mé bakalářské práce je "Vliv kontinuálního monitoringu glukózy na kompenzaci diabetu u dětí" Ve své práci jsem se zaměřila na využití systému kontinuální monitorace glukózy (CGM) a jeho přínosu, pro lepší kompenzaci u dětských pacientů. Poukázat na výhody i možné nevýhody jeho užívání. A alespoň trochu Vám přiblížit život diabetických dětí a jejich rodin. Teoretická část, je zaměřena na onemocnění Diabetes Mellitus 1 typu. Je popsána definice, příznaky, diagnostika i možnosti léčby, ve které se zaměřím především na využití inzulínové pumpy spolu s využitím kontinuální monitorace glukózy (CGM) a v neposlední řadě i na komplikace, které sebou toto onemocnění nese. Je zde popsána ošetřovatelská péče o diabetické dítě v domácím prostředí. Empirická část je vypracovaná formou dvou kazuistik diabetických dětí, sledovaných v dětské diabetologické ambulanci. U obou dětí, je od záchytu diabetu podobný postup léčby. Kdy od léčby inzulínovými pery se přešlo k léčbě inzulínovou pumpou a užívání systému kontinuální monitorace (CGM) U obou dětí lze vypozorovat postupné zlepšení kompenzace a to hlavně na základě laboratorních výsledků HbA1c (glykovaný hemoglobin) Dalším ukazatelem jsou hodnoty glykémií, které jsou průběžně zaznamenávány do diabetických deníků dětí a data získaná z CGM. Křivky a získaná data naleznete v přílohách. Cílem a úkolem bakalářské práce, je poukázat na přínos užívání systému CGM u dětí, kde bývá kompenzace diabetu obtížná. Dále na nutnost důsledné a vlastně nikdy nekončící edukace a v neposlední řadě i na zodpovědnou spolupráci jak rodičů, tak i malých diabetiků.

Abstract:

The topic of my bachelor thesis is the influence of continual monitoring of glucose on a compensation of diabetes in juvenile patients. In my thesis I concentrated on the use of the system of continual monitoring of glucose /CGM/ and its contribution to the improvement of compensation in juvenile patients. I also wanted to point at some advantages and possible disadvantages of CGM implementation. Last but not least I tried to describe everyday life of children, suffering from diabetes, and their families. My thesis is divided into two parts - theoretical and empirical. The theoretical part deals with diabetes mellitus type I. It provides its definition, symptoms, diagnosis and possibilities of treatment. Above all I tried to explain the use of a portable insulin pump which helps to realize continual monitoring of glucose /CGM/, and I also tried to point at some possible complications caused by this disease. The empirical part consists of two case studies dealing with a long term monitoring of two juvenile patients who have been under permanent watch in a juvenile diabetes ward. Both children undergo a similar way of treatment since their disorder has been diagnosed. Since insulin pumps instead of insulin pens have been implemented with both children, and a system of continual monitoring was installed, I could observe gradual improvement in compensation which was proved by laboratory results of HbA1c /glucose hemoglobin/ and other indicators, such as the level of hypo or hyperglycemia which is regularly marked in juvenile patients' diaries, or by data received from CGM. Main goals of my bachelor thesis were as follows: Firstly, to point at advantages of the system CGM implemented in monitoring of juvenile patients with whom the compensation of diabetes is difficult; Secondly, to stress the necessity of never ending education in the field of diabetes; Thirdly, the importance of responsible cooperation of parents and juvenile patients with physicians.
