Costs of Alzheimer disease in the Czech Republic using COI methodology

Abstract:

Alzheimerova choroba je velice diskutované téma, a to z několik důvodů – důsledky na pacienta z medicínského pohledu, ze sociálního pohledu, kde je nutnost zainteresování neformálních pečovatelů/ jiných sociálních služeb (ambulantní pobytové a terénní služby) a stoupající ekonomické dopady (způsobené růstem počtu osob a prodlužujícím se věkem). Jedná o nevyléčitelné degenerativní in-reverzibilní onemocnění postihující mozek, při kterém dochází k postupnému odumírání mozkové tkáně a snížení počtu neuronů. To má za následek výpadek a zhoršení paměti, snížení až téměř vymizení kognitivních, fyzických a intelektuálních schopností. Dle proběhlých výzkumů se péče o pacienta s AD považuje za velmi nákladnou. Kdy finanční nákladovost roste s progresí nemoci dle jednotlivých fází. Cílem této diplomové práce je výpočet nákladů Alzheimerovy choroby pomocí metodiky Cost-of-illness, která stojí na třech hlavních pilířích. První pilíř jsou dotazníky, které byly vytvořeny na projekt TAČR. Dále pak case study vybrány tak, aby reprezentovaly každý stupeň onemocnění. Třetím pilířem jsou řízené rozhovory, kde jsou doplňující informace, které jsou potřebné k metodice Cost of illness (dále pak COI), která tvoří metodický rámec celé práce. Pro výzkum byla využita data z Národního registru hrazených zdravotních služeb poskytnuté z ÚZIS a data přímo od respondentů. Hlavním přínosem diplomové práce je využití pro další rozpracování této problematiky a podmět pro další výzkumné šetření.

Abstract:

Alzheimer´s Disease (AD) is an incurable, degenerative, irreversible health condition affecting memory and thinking abilities due to neuron’s reduction. AD results in memory loss and deterioration, as well as other cognitive, physical and intellectual abilities being reduced nearly to extinction. These losses affect the ability to carry out the simplest tasks. AD is a severe health condition bringing burden not only to the patient but to health and social systems and, especially, to families who become informal and, often, the predominant caregivers. Research has shown that care for an AD patient is very costly. The cost increases with the progression of the disease through its progressive its phases. The aim of this research article is to calculate the costs of Alzheimer's disease using a framework based on cost-of-illness methodology. The data are based on four main pillars. The first is a caregiver’s questionnaire, the second, the patients´ case studies selected to represent each stage of the disease, the third is in-depth interviews with the patients and caregivers, while the fourth is data from the Institute of Health Information and Statistics of the Czech Republic (UZIS) which provides data of health care consumption.
