Attitude of midwives to outpatient delivery and follow-up care after delivery

Abstract:

Cíl: Hlavním cílem diplomové práce bylo zjistit a popsat postoj porodních asistentek k ambulantnímu porodu a následné péči po ambulantním porodu. Metodika: Pro přesné vystihnutí názorů porodních asistentek na ambulantním porodu a následnou péči po porodu byl zvolen kvalitativní výzkum. Porodní asistentky byly vybrány záměrně. Výzkumný soubor tvořilo osm porodních asistentek, čtyři z neonatologického oddělení a čtyři z porodního sálu dvou porodnic. Výzkum vycházel z kvalitativní analýzy dat, ze kterých jsme porozuměli výpovědím porodních asistentek. Výsledky: Výzkumem bylo zjištěno, že porodní asistentky ve většině se staví kladně k ambulantnímu porodu. Dvě porodní asistentky mají spíše negativní postoj, ale respektují přání ženy. Zajímal nás přehled porodních asistentek o následné péči v domácím prostředí, především, jak funguje porodní asistentka v šestinedělí a zajištění pediatra po ambulantním porodu. V péči o ženu v šestinedělí mají porodní asistentky přehled, o následné péči o novorozence dvě porodní asistentky nemluvily, protože si nebyly jisty, jak přesně péče probíhá. Závěr: Porodní asistentky by neměly problém s ambulantními porody, kdyby byla zajištěná komplexní a pro všechny dostupná péče o matku a novorozence. Strach z dřívějšího odchodu z porodnice mají především z komplikací jak u novorozence, tak u matky, které nemusí žena sama včas odhalit.

Abstract:

Objective: The main aim of master thesis was to identify and desribe attitude of midwives for ambulant delivery and outpatient follow-up care after birth. Methodology: A qualitative research was chosen for capturing the opinions of midwives. The midwives of the research were chosen intentionally. The exploratory group was composed of eight midwifes, four of neonatal care unit and four of birth hall of two maternity hospitals. The research was based on a qualitive analysis of data from which can understand the statements midwives. Results: The research found that midwives in the most have a positive approach to ambulant delivery. Two midwives have a rather negative attitude, but respect the wishes of women. We interested in on overview of midwives aftercare at home, especially how midwife work in puerperium and ensure pediatriac after ambulant delivery. Midwives have a goood overview about care of women in puerperium. Two midwives did not speak about neonatal care becouse they are not sure how exactly the treatment take place. Conslusion: Midwifes should not a problem with ambulant delivers if it was secured complex and available care for mother and newborn. Midwives have a fear of complication of the newborn and mother after ambulant delivery. Women does not detect complicatin herself.
