Coping strategies of therapists working with depressed clients

Abstract:

Tato práce zkoumá zvládající strategie terapeutů během terapie s depresivními klienty, která pro terapeuty často představuje velkou zátěž. Terapeuti jsou ohroženi přenosem depresivních prožitků, ale i pocity frustrace a viny a následným narušením vlastního duševního zdraví. Prezentovaná studie se proto zaměřuje na zvládající strategie, které terapeuti využívají během terapeutického sezení s klientem tak, aby nebyli vtaženi do deprese a zároveň v terapii s klientem vydrželi. Metodou tvorby dat byly individuální rozhovory a ohniskové skupiny se 30 terapeuty různých psychoterapeutických škol. Data byla následně analyzována pomocí metody zakotvené teorie. K analýze dat byl použit konstruktivistický přístup K. Charmazové. Na základě analýzy dat byly identifikovány základní kategorie zvládajících strategií, které terapeuti využívají v průběhu terapeutického sezení. Základní zvládající strategie byly konceptualizovány jako Introspektivně zaměřené zvládající strategie a Extrospektivně zaměřené zvládající strategie, které vystihují hlavní procesy, ke kterým dochází v průběhu zvládání. Terapeuti obrací pozornost vně nebo dovnitř. Strategie zaměřené vně umožňují terapeutům aktivně ovlivňovat daný stresor, dodávají jim větší pocit kontroly a umožňují poodstoupit se od vlastních negativních prožitků. Strategie zaměřené dovnitř přispívají k větší sebereflexi a přehodnocení vlastní pozice a možností v terapii. Kromě těchto strategií byly identifikovány i mimoterapeutické zvládající strategie. Výsledky výzkumu prezentují hlavní zvládající strategie, které umožňují terapeutům vydržet psychickou zátěž během terapie.

Abstract:

This paper explores therapists‘ coping strategies used during therapy with depressive clients which is a big burden for therapist. Therapists are at risk for the transmission of depressive experiences, but also for the feelings of frustration and guilt and the consequent disruption own mental health. The present study therefore focuses on coping strategies used by therapists during a therapy session with the client, which help not to be drawn into a depression and endured while in therapy with the client. Data were gained from individual interviews and focus groups with 30 therapists of various schools of psychotherapy. Data were consequently analyzed using grounded theory method. Data were analyzed using the constructivist approach of Charmaz. Based on data analysis basic categories of coping strategies used during the therapeutic session were identified. The basic coping strategies were conceptualized as Introspective focused coping strategies and Extrospektive focused coping strategie. They describe main processes that occur during therapy management. Therapists turns their attention outwards or inwards. Strategies aimed outside allow therapists to actively influence the stressor, provide a greater sense of control and allow them to step back from their own negative experiences. Strategies aimed inward contribute to greater self-reflection and reframe their position and opportunities in therapy. Besides these strategies have been identified beyond – session therapeutic coping strategies. The results of the research presents major coping strategies that allow therapists to endure psychological burden during therapy.
