Circulating tumor cells in solid tumors.

Abstract:

Diplomová práce se zaměřuje na charakterizaci nádorového onemocnění prostřednictvím cirkulujících nádorových buněk (circulating tumor cells, CTC). V rámci experimentální části diplomové práce byly CTC buňky izolovány z periferní krve pacientů s urogenitálními tumory metodou závislou na velikosti buněk (tzv. size based separation) a charakterizovány z hlediska cytomorfologie a molekulární biologie. CTC buňky byly kultivovány in vitro a podrobeny in vivo mikroskopické analýze pomocí vitální fluorescenční mikroskopie a následně zobrazeny pomocí konfokální a rastrovací elektronové mikroskopie. Pro analýzu exprese genů ve frakci CTC buněk byla použita kvantitativní polymerázová řetězová reakce (qPCR). Analýza prostřednictvím metody qPCR zahrnovala tzv. tumorasociované geny. Získaná data byla vyhodnocena vzhledem ke standardizaci užitých metod a jejich potenciálního přínosu do klinické laboratorní praxe. Celkem byl výskyt CTC buněk hodnocen u 65 vzorků pacientů s převážně urogenitálními tumory. Bylo použito 5 zobrazovacích a detekčních metod. CTC buňka u karcinomu ledviny dosahuje průměrné velikosti jádra 11,08?2,421 ?m (9 12 ?m/n=193) a průměrné velikosti buňky 29,81?10,76 ?m (21 36 ?m/n=193). Průměrná velikost jádra CTC buněk u pacientů s karcinomem prostaty dosahuje 10,58?1,834 ?m (9 13 ?m/n=387) a průměrná velikost buňky 27,21?7,511 (21 42 ?m/n=387). Následná qPCR analýza potvrdila zvýšenou expresi některých tumorasociovaných genů v CTCobohacené frakci v porovnání s jejich expresí v periferní krvi. Kombinace testování exprese genů EPCAM, KRT18, KRT19 a OR51E2 by mohla být v budoucnu využita v rámci standardizovaných panelů genové exprese pro identifikaci CTC buněk u karcinomu prostaty.

Abstract:

The presented diploma thesis focuses on the characterization of cancer via circulating tumor cells (circulating tumor cells, CTCs). CTCs were characterized in terms of cytomorphology and molecular biology. In the experimental part of the thesis CTC were isolated from peripheral blood of patients with urogenital tumors by a method dependent on cell size (ie. sizebased separation). CTCs were cultured in vitro and subjected to in vivo microscopic analysis using a vital fluorescent microscopy, and then displayed using confocal and scanning electron microscopy. For analysis of gene expression in the cell fraction of CTC quantitative polymerase chain reaction (qPCR) was used. Analysis using the method called qPCRbased analysis included tumorassociated genes. The obtained data were evaluated with respect to the standardization of the methodology used and its potential contribution to the clinical laboratory practice. Total incidence of CTC presence was studied in 65 samples of patients with urogenital tumors. 5 imaging and detection methods were used. CTC in renal carcinoma reaches an average nucleus 11.0?2.421 microns (912 ?m/n = 193) and the average cell size of cell 29.8?10.76 ?m (21 to 36 ?m/n = 193). The average size of the nucleus of CTC in patients with prostate cancer reaches 10.58?1.834 ?m (913 ?m/n = 387) and the average cell size reaches 27.21?7.511 ?m (21 to 42 ?m /n = 387). Subsequent qPCR analysis confirmed the overexpression of certain tumorassociated genes in the CTCenriched fraction as compared to their expression in peripheral blood. The combination of testing EPCAM, KRT18, KRT19 OR51E2 by qPCR and could be used in future as part of standardized panels of gene expression for identification of CTC in prostate cancer.
