Clinical examination methods in the diagnosis of carpal tunnel syndrome

Abstract:

Abstrakt Tato práce se zabývá možnostmi vyšetření a diagnostiky syndromu karpálního tunelu. Obecná část je věnována anatomii ruky a karpálního tunelu, neurofyziologickým informacím vedení vzruchů v nervech, charakteristice syndromu karpálního tunelu, diagnostickým metodám a prevenci syndromu. V praktické části se potom pokusím zhodnotit které z testů v jednotlivých vyšetřovaných oblastech jsou pozitivní a tedy průkazné a které nikoliv. Cílem práce je hodnotit četnost pozitivních a negativních odpovědí pacienta a jeho organismu na testování v jednotlivých oblastech zasažených při syndromu karpálního tunelu. Pro vypracování práce jsem použila metodu ošetřovatelského kvalitativního výzkumu. Data pro vypracování práce jsem získal z oborných dokumentů, dotazováním, na ty části vyšetření závislé na pacientově subjektivním prožívání, pozorováním a testováním v těch částech vyšetření které toto vyžadují. Odebraná vyšetření obsahují testované kvality členěné do jednotlivých celků. Nebyly odebrány kazuistiky jako takové. Údaje potřebné pro tuto práci poskytuje vyšetření, viz přílohy. Oblasti vyšetření které jsem zkoumal jsou : anamnéza, charakter obtíží, subjektivní pocity, bolest, parestézie, jemná motorika, specifické provokační testy, vyšetření povrchového čití, vegetativní symptomatika, hypotrofie, parézy, pasivní pohyb a aktivní pohyb. Některá vyšetření z oblasti specifických provokačních testů byla nepříjemná či mírně bolestivá. Popsané metody, především ze skupiny provokačních testů, pravděpodobně nebudou masověji užívány pro svou relativní nespolehlivost, tedy vyšší než přiměřenou četnost falešně pozitivních nebo negativních výsledků. Práce si tedy neklade nároky či očekávání zanícenějšího používání oněch testů, je jen jakousi ilustrací možností, které moou či nemusí potvrdit diagnózu SKT.

Abstract:

Abstract This work deals with the possibilities of examination and diagnosis of carpal tunnel syndrome. The general part is devoted to the anatomy of the hand and carpal tunnel neurophysiological information conduction in the nerves characteristics of carpal tunnel syndrome, diagnostics and prevention syndrome. In the practical part then try to assess which of the tests in each of the investigated areas are positive and therefore conclusive and which are not. The aim is to evaluate the frequency of positive and negative responses of the patient and his body for testing in different areas affected in carpal tunnel syndrome. For the development work I used the method of nursing qualitative research. Data for the development work, I got out of oborných documents, questioning, those parts of the examination depends on the patient's subjective experience, observation and testing in those parts of the examination requiring this. Removed tests include testing the quality broken down into individual units. Case reports were taken as such. The data required for this work provides examinations, see Annex. Areas of examination that I have examined are: history, character problems, subjective feelings, pain, paresthesia, fine motor skills, specific provocation tests, examinations surface sensation, autonomic symptomatology, wasting, paresis, passive movement and active movement. Some tests of area-specific provocation tests was slightly uncomfortable or painful. These methods, especially from provocation tests unlikely pervasive used for their relative unreliability, therefore more than adequate frequency of false positive or negative results. So the paper does not claim or expectation zanícenějšího use those tests, it is only a kind of illustration of the possibilities moou or may not confirm a diagnosis of CTS.
