A Systems Approach to Health Care Financing

Abstract:

Tato diplomová práce se zabývá českým zdravotním systémem. Zdravotnictví je obecně nazýváno jako systém, ale ne vždy jsou jeho problémy řešeny systémově. V práci je proto popsána systémová teorie, teorie komplexně adaptivních systémů a další pojmy, které se ve zdravotnictví často používají, ale jejich potenciál není využit tak, jak by měl. Cílem této práce bylo vysledovat dopady případné spoluúčasti pacientů (20 %, 25 % a 30 %) na finanční situaci českých domácností podle ukazatelů chudoby. Ukazatele chudoby definovala Světová zdravotnická organizace (WHO) pro Výzkum národního zdraví v jednotlivých zemích. Jedná se o čtyři základní ukazatele - hranici chudoby, existenční minimum, počet zbídačených domácností a domácnosti ohrožené katastrofickými výdaji na zdraví. Tyto ukazatele byly aplikovány na vzorek českých domácností a byly sledovány dopady zvýšené spoluúčasti domácností. Výsledky ukázaly, že na finanční situaci (dle ukazatelů chudoby) daného vzorku domácností by zvýšená spoluúčast neměla prakticky žádný dopad. Jen velmi málo domácností by se ocitlo na hranici chudoby. Nicméně, výsledky se nedají zobecnit na všeobecnou populaci České republiky a s ukazateli by se mělo ještě dále pracovat a rozvíjet je, aby mohly být aplikovány a mohly tak sloužit jako jeden z indikátorů pro sledování dopadů určitých zásahů do zdravotního systému.

Abstract:

This diploma thesis deals with the Czech healthcare system. Healthcare is generally reffered to as system but not always healthcare problems are solved systematically. Therefore systems theory, theory of complex adaptive systems and other notions, that are often used in healthcare, were described in this thesis. The objective of this thesis was analysis of impacts of patient's financial participation (20 %, 25 % and 30 %) on health care to their financial situation in the complex adaptive healthcare system. In this thesis four indicators were used -- poverty line, household subsistence spending, impoverished households, catastrophic health expenditure. The World Health Organization defined this indicators for World Health Survey in different countries. The indicators were applied to a sample of Czech households and some impacts of increased participation were calculated. It results from this calculation that the increased participation of households has virtually only small impacts to their financial situation. By the indicators of poverty just few households would be on the poverty line. Nevertheless, this results aren't generalized to the whole Czech population. With this indicators we should further work and develop them in order that they could serve as tool to evaluation of state's intervetion into healthcare system.
