Documentation of independent midwives

Abstract:

The work analyzes the history of recordkeeping in the midwifery practice, the current situation in documenting the care of a woman during pregnancy, childbirth and postpartum, as well as legislative regulations in this field. There are surprisingly large differences in the opinions on the timing and content of the screening is believed to provide benefits. While the healthcare system in the Czech Republic expects a high degree of compliance from women, many foreign healthcare systems are more focused on women and involve her in decisions about provided healthcare. The practical outcome of this work is the proposal of specific medical records for the needs of independent midwives. Forms are designed for three larger, thematically related units (antenatal documentation, birth record, and postpartum documentation), for which the ideal size of the worksheet is A3 format and additional documents optimized for A4 format, printed and used if necessary. Testing the model, reevaluating it and putting it into practice could help to unify the healthcare system, improve communication between health professionals and form the basis for statistical data collection.

Abstract:

Práca analyzuje históriu zaznamenávania informácií v pôrodnej praxi, súčasnú situáciu v dokumentácii pôrodu a starostlivosti o ženu v tehotenstve a po pôrode ako aj legislatívne predpisy v tejto oblasti. Medzi názormi ohľadom počtov a obsahu vyšetrení, ktoré poskytujú žene úžitok existujú prekvapivo veľké rozdiely. Kým zdravotný systém v Českej republike očakáva vysoký stupeň konformity žien s predpísaným spôsobom zdravotnej starostlivosti, mnohé zahraničné zdravotné systémy sa viac sústreďujú na ženu a zapájajú ju do rozhodnutí o jej vlastnej zdravotnej starostlivosti. Praktickým výstupom tejto práce je návrh špecifickej zdravotníckej dokumentácie pre potreby samostatných pôrodných asistentiek. Navrhnuté formuláre sú koncipované do troch väčších, tematicky príbuzných celkov (dokumentácia rodičky, dokumentácia pôrodu a dokumentácia v šestonedelí) pre ktoré je ideálny rozmer listu formát A3 a prídavných dokumentov optimalizovaných pre formát A4, ktoré je možné v prípade potreby vytlačiť a doplniť do dokumentácie. Otestovanie vytvoreného modelu, jeho korekcia a uvedenie do praxe by mohlo prispieť k zjednoteniu systému poskytovania starostlivosti, zlepšeniu komunikácie medzi zdravotníkmi a vytvoriť základ pre štatistický zber údajov.
