Long-term maintenance program for an elderly patient with rheumatoid arthritis: a clinical case

Abstract:

In recent decades life expectancy has increased and in pace with this, even the quality of life, the elderly is no longer enough to go on with the years but, rightly, claims to continue to do the same things that he did before or otherwise to have an active life, to walk, to do sports, to travel, so diseases such as rheumatoid arthritis or traumatic events such as femur are approached with more attention and care now than in the past. Once the elderly subject with a fracture of the femur often became a bedridden subject and his quality of life was limited. Today, thanks to the surgical operations and the applications of prostheses and to appropriate physiotherapy treatments, the elderly or the patient in general is able to have even a better quality of life than before. The interest for this new way of thinking is the reason why we wanted to treat this topic in this thesis and why we wanted to expose the case of a patient suffering from rheumatoid arthritis that, for an accidental traumatic event went through a fracture of the femur and then a subsequent hip replacement surgery followed by forty day rehabilitation in a nursing home. The goal for which we worked in these forty days was not only to reduce the traumatic effect, but also to return to a good level of quality of life by reducing pain, increasing the ROM (range of movement) and strengthening the hip muscles so as to allow the patient to leave the clinic with crutches, knowing that a complete rehabilitation program for achieving most functional performance is six months.

Abstract:

In questi ultimi decenni l'aspettativa di vita ? aumentata e, di pari passo con questa, anche la qualit? della vita, alla persona anziana non basta pi? andare avanti con gli anni ma, giustamente, pretende di continuare a fare le stesse cose che faceva prima o comunque avere una vita attiva, camminare, fare sport, viaggiare, per cui malattie come l'artrite reumatoide o eventi traumatici come le fratture del femore vengono, ora, affrontati con un'attenzione e una cura maggiore che in passato. Una volta l'anziano soggetto ad una frattura del femore diventava, spesso, un soggetto allettato e la sua qualit? di vita veniva limitata. Oggi, grazie agli interventi chirurgici e alle applicazioni di protesi e ad opportune cure fisioterapiche, l'anziano o il paziente in genere, riesce ad avere addirittura una qualit? di vita migliore rispetto a prima. L'interesse per questo nuovo modo di ragionare ? il motivo per cui si ? voluto trattare questo argomento in questa tesi e per cui si ? voluto esporre il caso di una paziente affetta da artrite reumatoide che, per un evento traumatico accidentale, ? andata incontro a una frattura del femore e, quindi, ad un conseguente intervento chirurgico di protesi d'anca, seguito da una riabilitazione di quaranta giorni in una casa di cura. L'obiettivo per il quale si ? lavorato in questi quaranta giorni ? stato non solo quello di ridurre l'effetto traumatico, ma anche quello di tornare ad un buon livello della qualit? di vita, andando a ridurre il dolore, aumentando il ROM (range of movement) e rafforzando i muscoli dell'anca in modo tale da permettere al paziente di uscire dalla clinica con le stampelle, sapendo per? che un programma completo di riabilitazione per il raggiungimento della maggior parte delle prestazioni funzionali ? di sei mesi.
