Detekce lidského cytomegaloviru u pacientů po transplantaci hematopoetických kmenových buněk

Detection of human cytomegalovirus in patients after hematopoetic stem cell transplantation

Abstract:

Metoda polymerázové řetězové reakce (Polymerase Chain Reaction, PCR) je schopná detekovat specifickou sekvenci virového genomu v klinickém vzorku. PCR je obecně dostatečně citlivá metoda, kdy teoreticky i z jedné kopie HCMV genomu je možno pomocí PCR vytvořit dostatečné množství specifického produktu, který jsme schopni zachytit. K detekci HCMV jsme použili metody dvoukolové nested PCR, reverzně transkripční PCR (RT-PCR) a jednokolové PCR. Po zhodnocení jejich výhod a nevýhod pro klinickou rutinní diagnostiku u pacientů po alogenní HSCT byla zvolena metoda jednokolové PCR. Byla potvrzena její vysoká specificita a dostatečná citlivost. Pacientům po alogenní HSCT byl pravidelně jednou týdně odebírán vzorek periferní krve (Peripheral Blood, PB). První odběr byl proveden v den 0 - 7 po HSCT. Ze vzorku byly izolovány leukocyty (Peripheral Blood Leukocytes, PBL) a z nich následně DNA, která byla testována na přítomnost HCMV-DNAémie. Do retrospektivního hodnocení klinického testování byly zahrnuty výsledky testování 71 pacientů po alogenní HSCT. Celkově bylo provedeno 73 sledování, protože u dvou pacientů byla provedena retransplantace. Medián doby sledování byl 313 dní. Alespoň jeden pozitivní HCMV-PCR výsledek byl prokázán u 41/73 sledování (56,1%). U 28/73 sledování (38,4%) byla zachycena HCMV-DNA ve dvou po sobě následujících týdnech. Opakovaná detekce HCMV-DNA byla indikací pro nasazení preemptivní terapie. U 4/73 (5,5%) sledování byla zachycena jedna rekurence HCMV infekce a u jednoho sledování byly zachyceny dvě rekurence (1,4%). Metoda kvalitativní jednokolové PCR detekce zachytila včas a rychle reaktivaci HCMV. Protože výsledek PCR detekce byl znám stejný den po dodání vzorku PB do laboratoře nebo do druhého dne při potvrzení pozitivního výsledku, bylo možné brzy rozhodnout o nasazení preemptivní terapie. U žádného pacienta nedošlo během sledování k rozvoji HCMV onemocnění.

Abstract:

Human cytomegalovirus (HCMV) is a wide-spread pathogen. After primary infection, which is in immunocompetent person mostly asymptomatic, the virus persists in the host in a latent stage. HCMV infection in an immunocompromised person often evokes serious clinical complications (leucopenia, thrombocytopenia, acute chorioretinitis, encephalitis, hepatitis, myocarditis, pancreatitis, pneumonia). It may be cause of rejection of transplant organs or development of the graft versus host disease (GvHD) after transplantations. HCMV infection and disease may result in the risk of second infection with other pathogens. Preventive arrangements and antiviral treatments decrease HCMV mortality and morbidity. If the antiviral drug ganciclovir (GCV) is administered to patients before the development of HCMV disease, HCMV responds very well to the treatment and the disease does not occur in a major part of cases. GCV may be administered to patients prophylactically, i.e. during a distinct time interval to all patients with the risk of development of the disease, or selectively (preemptive therapy), i.e. to a defined group of patients on the basis of a positive detection of sufficiently sensitive and specific marker of the reactivation of HCMV. However, rophylactic use of GCV has also side effects, such as neutropenia (reduced number of neutrophiles) and myelosuppression (suppression of hematopoesis in the bone marrow), with accompanying increased risk of occurrence of secondary infections with frequent fatal consequences in immunocompromised patients. The doctoral thesis proposed three goals: to validate and compare the available molecular biological methods for an early detection of reactivation of HCMV (DNAemia), to select a suitable method from point of view of routine clinical monitoring of hematooncological patients after allogeneic stem cell transplantation (HSCT), and to validate the chosen method from the standpoint of the results of the preemptive antiviral therapy. The detection method must be able to detect the virus in a clinical sample of patients sufficiently early before the development of clinical complications induced by HCMV infection. Quick obtaining of results is important for early administration of the preemptive therapy. The method must be easy to execute and able to process several samples at the same time.
