The role of serum vitamin D levels on multiple sclerosis progression

Abstract:

Úvod: Roztroušená skleróza je vážné autoimunitní onemocnění centrálního nervového systému. Prvotní příznaky se objevují ve vysoce produktivním věku, častěji jsou postiženy ženy. Etiologie onemocnění není zcela přesně známa, z neinfekčních faktorů je však role vitaminu D nesporná. Nízké sérové hladiny vitaminu D nepředstavují pouze riziko pro rozvoj roztroušené sklerózy, ale vitamin D díky svým imunomodulačním účinkům pravděpodobně dokáže i efektivně ovlivnit prognózu již probíhajícího onemocnění. Teoretická část práce úzce navazuje na práci bakalářskou, shrnuje její poznatky a nabízí další informace k problematice roztroušené sklerózy. Segment teoretické části práce se věnuje vitaminu D a jeho imunomodulačním schopnostem. Praktická část práce se věnuje významu sérových hladin vitaminu D na progresi roztroušené sklerózy. Metodika: Ve studii bylo monitorováno celkem 106 pacientů (73 žen a 33 mužů) léčených v RS centru ve Fakultní nemocnici Ostrava. Základním diagnostickým kritériem byl klinicky izolovaný syndrom, nebo relaps-remitentní roztroušená skleróza. Pacienti byly sledováni po dobu 5 let od zahájení terapie léky 1. linie. Data byla získávána z nemocničního programu IKIS, poté byli pacienti rozděleni podle unikátní soustavy bodů do 13 skupin, kde finální bodové skóre symbolizovalo tíži progrese onemocnění, tedy vyšší počet bodů poukazoval na horší prognózu. K analýze získaných dat byla použita základní popisná statistika, Studentův t-test a neparametrický Wilcoxonův test. Testy byly vyhodnoceny na 5% hladině významnosti. Výsledky: Ve hladinách vitaminu D nebyl mezi pohlavími pozorován rozdíl (p=0,3201). Pacienti eskalující v terapii měli obvykle nižší hladiny vitaminu D. Průměrné hladiny vitaminu D dosahujících normy měly protektivní charakter vůči eskalaci, tedy i progresi (OR= 0,17; IS 95 % <0,04; 0,79>). Mezi skupinou s horší prognózou a skupinou s lepší prognózou nemoci pozorujeme signifikantní rozdíl průměrných hladin vitaminu D (p <0,001). Vyšší průměrné hladiny vitaminu D souvisely se stabilním stavem, ojediněle i se zlepšením, naproti tomu nízké hladiny predikovaly horší prognózu onemocnění. Závěr: Vitamin D má zásadní vliv na prognózu roztroušené sklerózy a mělo by být našim cílem dosáhnout jeho optimálních sérových hladin nejen u pacientů s RS.

Abstract:

Introduction: Multiple sclerosis is severe autoimmune disease of central nervous system. The first symptoms appear in productive age, women being affected more often. The etiology remains incompletely understood, though vitamin D is undisputedly very important non-infectious risk factor. A low serum vitamin D level is not only the risk factor for developing multiple sclerosis, but it can probably positively affect the prognosis of the disease thanks to its immunomodulating effect. The theoretical part is tightly connected with the bachelor thesis. Part of it is devoted to the vitamin D and its immunomodulating effects. The practical part studies the effect of low serum vitamin D levels on multiple sclerosis progression. Methods: We examined a total of 106 patients (73 females and 33 males) with either clinically isolated syndrome or relapsing-remitting MS who were treated in MS center of University Hospital in Ostrava. The follow-up period was 5 years since starting 1st line therapy. The data were gathered from the hospital software called IKIS. The patients were then divided into 13 groups according to the point system, where the final score represented the severity of disease progression, with more points meaning poorer prognosis. We analyzed the data using Student´s t-test and non-parametrical Wilcoxon test. The tests were evaluated at the significance level of 5%. Results: We did not observe significant differences in vitamin D serum levels between genders (p=0.3201). Patients who needed escalation treatment usually had lower vitamin D serum levels. Average normal vitamin D serum levels were protective against escalation, and hence the severity of the disease (OR= 0.17; IS 95 % <0.04; 0.79>). There was a significant difference in average vitamin D serum levels between the group with the best and the worst prognosis (p<0.001). Higher average vitamin D serum levels were associated with the stable state, sometimes even with improvement. Lower average vitamin D serum levels were on the other hand associated with poorer prognosis of the disease. Conclusion: Vitamin D has significant effect on multiple sclerosis prognosis and it should be our common goal to reach its optimal serum levels (not only) in patients with MS.
