The issue of treatment of patients with percutaneous endoscopic gastrostomy

Abstract:

Tématem Bakalářské práce je Problematika ošetřování pacientů s perkutánní endoskopickou gastrostomií. Hlavním cílem je zjistit rozsah znalostí sester při ošetřování pacientů s perkutánní endoskopickou gastrostomií /dále jen PEG/. Pro splnění hlavního cíle byly zvoleny i dílčí cíle: Zjistit, zda sestry mají dostatečné praktické dovednosti při ošetřování pacientů s PEG. Zjistit, zda sestry znají vhodný typ výživy do PEG. Zjistit, zda sestry znají komplikace po založení PEG. Teoretická část popisuje především perkutánní endoskopickou gastrostomií, ale i stomii obecně, vč. historie. Dále jsou zde uvedené indikace a kontraindikace u zavedení, způsoby, metodami, přípravou pacienta před výkonem a následné ošetřování pacienta se zavedenou perkutánní endoskopickou gastrostomií. Poté jsou popsány komplikace po zavedení PEG, správné postupy aplikace výživy a léků, odstranění či výměna stomického katetru. Poslední kapitola popisuje enterální výživu, zde jsou zahrnuty vlastnosti, zásady, přípravky a komplikace enterální výživy. Praktická část byla zpracována a vyhodnocena na základě dotazníkového šetření. Cílový vzorek respondentů tvořilo 112 sester z Masarykovy nemocnice, Domovů důchodců, Agentur Domácí péče a Nemocnice následné péče Ryjice. Každá otázka z dotazníku je pro přehlednost zpracována do tabulek a grafů. Z dotazníkového šetření vyplívá, že ošetřovatelská péče o pacienty s perkutánní endoskopickou gastrostomií je specifická, sestry se tuto novou způsobilost musí naučit. Je nutné, aby si sestry osvojily nové teoretické znalosti a získaly praktické dovednosti, vztahující se k této problematice. Jde např. o odlišný způsob při podávání stravy, volby vhodné výživy či péči o stomický katetr nebo řešení vzniklých komplikací. Do procesu je třeba zapojit i pacienta a jeho rodinu. Tito pacienti se mohou vyskytovat ve všech oblastech poskytování ošetřovatelské péče. Je pochopitelné, že i zde je potřeba chápat ošetřování pacienta komplexně, s holistickým přístupem. Výsledky šetření budou poskytnuty zařízení, ve kterém šetření probíhalo a mohou sloužit ke zlepšení ošetřovatelské péče o pacienty s perkutánní gastrostomií či k vytvoření nebo úpravě standardu ošetřovatelské péče o pacienty s PEG.

Abstract:

Bachelor thesis topic is The issue of treating patients with percutaneous endoscopic gastrostomy. The main objective is to determine the extent of knowledge of nurses in treating patients with percutaneous endoscopic gastrostomy /in short PEG/. To fulfill the primary targets were defined also sub-goals: To determine whether nurses have sufficient practical skills in treating patients with PEG. To determine whether nurses know complication after the introduction of PEG. The theoretical part describes especially percutaneous endoscopic gastrostomy, but also stoma in general, including the history. Furthermore, there are indications and contraindications with implementation, methods, techniques, preparation of the patient before the procedure and following treatment of the patient with a percutaneous endoscopic gastrostomy. Follows a description of complications after the introduction of PEG, proper procedures in the application of nutrition and medicines, removal or replacement of ostomy catheter. The last chapter describes enteral nutrition. There are included features, policies, products and complications of enteral nutrition. The practical part was processed and analyzed based on the survey. The target sample of respondents consisted of 112 nurses from Masaryk hospitals, nursing homes, home care agencies and Aftercare Hospital Ryjice. Each question from the questionnaire is for clarity processing in tables and graphs. The survey shows that the nursing care of patients with percutaneous endoscopic gastrostomy is specific. Nurses have to learn this new qualification and it is necessary to make new theoretical knowledge and gain the practical skills related to this issue. It's for example a different way of feeding diets, choice of appropriate diet and care for ostomy catheter or solutions developed complications. In the process, it is necessary to engage the patient and his family. These patients may be present in all areas of nursing care. It is understandable that there is a need to understand the complex treatment of the patient, with a holistic approach. Results of the survey will be provided to a medical facility in which the survey was done, and may serve to improve the nursing care of patients with percutaneous gastrostomy or to create or modify a standard of nursing care for patients for PEG.
