Relationship of ejaculate parameters and embryo anomalies detected using the preimplantation genetic diagnosis

Abstract:

Problémy s přirozeným otěhotněním mohou být založeny na širokém spektru příčin. Jedním z hlavních faktorů negativně ovlivňujících výsledek snahy o dosažení úspěšné gravidity jsou genetické abnormality vyskytující se v genomu embryí. Jejich původ může zahrnovat jak odchylky přinášené do vznikajícího embrya jednou či oběma gametami, tak chyby vznikající během dělení buněk již oplozeného vajíčka. Nezanedbatelnou část změn genetického materiálu přináší do nově vznikajících embryí spermie, ať už v průběhu přirozeného početí či po technikách mimotělního oplození. V rámci této práce jsou porovnány výsledky vyšetření pacientů kliniky reprodukční medicíny a preimplantační genetické diagnostiky Repromeda. Na jedné straně stojí data o výsledcích vyšetření ejakulátu 2721 mužů, na straně druhé výsledky genetického vyšetření 8450 embryí. U většiny skupin pacientů, jejichž embrya byla bioptována v třetí den po oplození, byl nalezen vztah mezi klesající kvalitou ejakulátu a zvýšeným výskytem genetických abnormalit v embryích. U embryí bioptovaných až ve stádiu blastocysty nebyla takto obecná tendence zpozorována.

Abstract:

Problems with natural conception can be based on a wide range of causes. One of the major factors negativelly affecting the pregnancy outcome are the genetic abnormalities found in the embryo genome. Their origin may include deviations brought into the embryo by one or both gamets or errors arising during the cell division. Sperm brings an appreciable part of the changes in genetic material to newly created embryos, whether during natural conception or after assisted reproductive technology. In this thesis are compared results of examinations of patients of the clinic of reproductive medicine and preimplantation genetic diagnosis Repromeda. On the one hand, there are data of the ejaculate examination results of 2721 men, on the other hand, there are results of genetic examination of 8450 embryos. In most of the categories of patients, whose embryos were biopted on the third day after fertilization, there was found a relationship between decreasing ejaculate quality and increasing incidence of genetic abnormalities in embryos. This tendency was not found in embryos biopted at blastocyst stage.
