Predictive Factors of Malignant Lymphomas - The Application of DNA Arrays in Lymphoid Malignancies

Abstract:

V předložené dizertační práci zpracovávám téma využití technologie DNA čipů (DNA mikročipy, DNA microarrays) u lymfoidních malignit. DNA čipy jsou zařízení schopná ve velmi krátkém čase paralelně detekovat a kvantifikovat expresi až desítky tisíc genů (stanovit profil genové exprese) a nebo identifikovat variabilitu genetického kódu. V první části práce se zaměřuji zejména na možnosti použití DNA čipů k identifikaci molekulárně definovaných entit uvnitř stávajících nosologických jednotek (molekulární taxonomie) lymfoidních malignit a klíčových molekul vhodných k jejich prediktivní diagnostice a cílené léčbě. Ve druhé části práce společně se svými spolupracovníky validujeme na modelu difúzního velkobuněčného B-lymfomu (DLBCL) prediktivní význam vybraných výsledků DNA čipových analýz, ať již vlastních, nebo publikovaných jinými autorskými týmy: 1. molekulární taxonomie DLBCL; 2. exprese beta-1 integrinu a vybraných regulátorů apoptózy beta-1 integrinové signální dráhy (Akt-kinasa, survivin); 3. exprese jednotlivých izoforem proteinkinasy C (PKC): PKC-betaI, PKC-betaII, PKC-gama, PKC-delta. V případě našeho souboru se nám podařilo potvrdit význam beta-1 integrinových receptorů, PKC-delta a antiapoptotických proteinů Akt kinasy a survivinu v predikci vývoje DLBCL. Naše pozorování jsou v souladu s biologickou funkcí uvedených prediktorů i s výsledky publikovanými u jiných typů primárních tumorů. Z tohoto pohledu je možné o těchto molekulách uvažovat jako o vhodných kandidátech k cílené protinádorové léčbě i u DLBCL. Naopak jsme vyvrátili informace o expresi PKC-gama u DLBCL, a upozornili jsme tak na některé nedostatky DNA čipových analýz a na nutnost ověřovat jejich výsledky nezávislou metodou.

Abstract:

The DNA microarrays are a device capable of rapid parallel detection and expression quantification of as many as tens of thousands of genes (gene expression profiling) or identification of the genetic code variability. The aim of my thesis was to process this new and progressively developing topic in lymphoid malignancies and, under clinical conditions, to validate both my own or other authors results of DNA microarray analyses using the model of diffuse large B-cell lymphoma (DLBCL). In the first part of the study, the DNA microarray applications are demonstrated in order to identify: 1) new entities defined on the molecular basis within the existing diagnostic (nosologic) units – a so called molecular classification (taxonomy) of the disease; 2) profiles of the gene expression predicting the development (prognostic profiles) and the response to the treatment (predictive profiles) in the existing diagnostic units; 3) signaling pathways and their key molecules that can be used for therapy – target therapy. In the second part of the study, the validity of the selected results of DNA microarray analyses is verified in a sample of patients with DLBCL: 1) the molecular taxonomy of the diffuse large cell B-lymphoma; 2) the expression of beta1-integrin and the selected regulators of apoptosis and its signaling pathway (Akt-kinase, survivin); 3) the expression of the individual iso-forms of protein kinase C (PCK) PKC-betaI, PKC-betaII, PKC-gamma, PKC-delta. Although not all the validated results of DNA microarray analyses were fully reproducible in our sample, the importance of beta1-integrin receptors, PKC-delta and antiapoptotic proteins of Akt-kinase and Survivin for the prediction of the DLBCL has been confirmed. On the other hand, the information on the PKC-gamma expression in DLBCL has been disproved and some drawbacks of DNA microarray analyses and the necessity of verifying their results by independent methods have been pointed out. We are aware of the fact that, with the limited number of the data published, no conclusions for the proteins mentioned can be drawn from our results. On the other hand, our results are in the accordance with their biological function and the results published on other types of primary tumours.
