Radionuclide esophageal transit study and its utilization in diagnostics of gastroesophageal reflux disease focussingon patients with bronchial asthma and chronic cough

Abstract:

Dynamická scintigrafie jícnu (DSJ) je jednoduchá, neinvazivní vyšetřovací metoda, která probíhá za fyziologických podmínek. Výhodou vyšetření je detekce poruch jícnové motility a záchyt gastroezofageálního refluxu (GER) v rámci jedné studie, tedy po jediné aplikaci radiofarmaka. Prokázaná porucha tranzitu polknutého bolusu radioaktivity jícnem ukazuje na sníženou clearanční ezofageální funkci a záchyt zpětného posunu radioaktivity ze žaludku do jícnu je známkou inkompetence dolního jícnového svěrače. Práce si klade za cíl zhodnotit význam DSJ v diagnostickém procesu vedoucímu k identifikaci osob s refluxní chorobou jícnu (RCHJ) se zaměřením na děti s bronchiálním astmatem a dospělé s bronchiálním astmatem a chronickým kašlem. Akvizici a vyhodnocení DSJ provádíme podle naší vlastní modifikace. Tekutý bolus radiofarmaka je podán vyšetřovanému per os pod detektorem gamakamery a po jeho polknutí hodnotíme tranzitní čas jícnem, antiperistaltiku a retenci radioaktivity v jícnu. Během druhé fáze studie detekujeme po dobu 30 minut scintigrafické známky GER. V rámci prvního cíle práce jsme u dětí i dospělých s bronchiálním astmatem prokázali signifikantně častější výskyt poruchy tranzitu jícnem a známek GER než u osob bez respiračního onemocnění. Zejména výskyt antiperistaltiky jícnu byl u dětí s astmatem významně častější než u dětí bez astmatu. Druhým cílem bylo obecné posouzení významu dynamické scintigrafie jícnu v diagnostice RCHJ. Podle našich výsledků se scintigrafické známky ezofageální dysmotility objevily u dospělých i dětských pacientů bez bronchiálního astmatu méně často než u nemocných s astmatem. Provedení DSJ však považujeme za přínosné pro svou diagnostickou informaci o stavu jícnové motility, která je hlavní determinantou očistné funkce jícnu. Proto považujeme zařazení této metody do diagnostického algoritmu choroby za plně odůvodněné.

Abstract:

Radionuclide esophageal transit study (RETS) is simple, noninvasive examination method, that is proceeded under physiological conditions. The advantage of RETS is detection of esophageal dysmotility and signs of gastroesophageal reflux (GER) during only one study, i.e. after single radiopharmaceutical administration. Abnormal transit of radioactivity through the esophagus gives the evidence of reduced clearance esophageal function and reflux of radioactivity from stomach into the esophagus is a sign of incompetence of lower esophageal sphincter. Our work aims to assess the significance of RETS in the diagnostic process leading to the identification of patients with gastroesophageal reflux disease (GERD), focusing on children with bronchial asthma and adults with bronchial asthma and chronic cough. Acquisition and processing of RETS we perform according to our own modification. Fluid bolus of radiopharmaceutical is given orally under gamacamera detector and after swallowing we evaluated mean transit time, antiperistalsis and retention of radioactivity in esophagus. During the second phase of the study we monitored over 30 minutes occurrence of signs of GER. According to the main objective of this work, children and adults with bronchial asthma showed a significantly higher incidence of scintigraphic signs of impaired esophageal motility and GER than in those without respiratory disease. Particularly, the incidence of esophageal antiperistalsis in children with asthma was significantly much more frequent than in those without asthma. Second objective was to assess the general importance of radionuclide esophageal transit study in diagnostics of GERD. According to our results, the evidence of esophageal dysmotility occurred in adult and pediatric patients without asthma less frequently than in patients with asthma. However, implementation of RETS we consider as beneficial for their diagnostic information about the state of esophageal motility, which is the main determinant of the purification function of the esophagus. Therefore, we regard the inclusion of RETS into the diagnostic algorithm of GERD as fully justified.
