Nursing Care of the Patient with Rhizatrose

Abstract:

Bakalářská práce se zabývá chirurgickým výkonem – rhisarthrosou, neboli artrózou kořenového kloubu palce ruky a ošetřovatelskou péčí o pacientku, která podstoupila operační léčbu rhisarthrosy. Cílem práce je detailně popsat případ 56leté pacientky, která je postižena primární artrózou prvního karpometakarpálního kloubu (M 180). V praktické části této práce je uvedena bezprostřední předoperační péče a dále zejména pooperační péče o pacientku zaměřená na management bolesti, posouzení místa chirurgického zákroku a bezpečnost pacientky. Pooperační péče je zaměřena na všechny fáze ošetřovatelského procesu: posouzení zdravotního stavu, ošetřovatelská diagnostika, stanovení očekávaných výsledků a plánu ošetřovatelské péče, realizace ošetřovatelské péče a její hodnocení. Údaje o pacientce byly získány z primárního zdroje – od pacientky samotné, dále byly využity sekundární zdroje – zdravotnická dokumentace a zdravotnický personál. Použitými technikami k získání dat byly: rozhovor, pozorování a měřící ošetřovatelské techniky. K posouzení zdravotního stavu pacientky byl použit ošetřovatelský model Marjory Gordonové, který zjistil funkční a dysfunkční vzorce zdraví a na jejich základě byly stanoveny ošetřovatelské diagnózy dle NANDA I Taxonomie II 2015-2017. Další fáze ošetřovatelského procesu – plánování, realizace a hodnocení jsou popsány u ošetřovatelských diagnóz Akutní bolest (00132), Riziko infekce (00004) a Narušený vzorec spánku (00198), což byly zásadní pacientčiny ošetřovatelské problémy v pooperační péči. Ošetřovatelská péče o pacientku byla efektivní, pacientka se cítila po celou dobu hospitalizace dobře, měla dostatek informací a byla dostatečně edukována. Artróza kořenového kloubu palce ruky je v současné době velkým problémem nejen pro starší věkové kategorie, ale i pro mladší ročníky. Jde o degenerativní postižení, jehož progresi lze jen stěží zpomalit, a to například aktivním cvičením kloubu, nebo minimálním zatěžováním kloubu. Chirurgickou léčbu volí lékař až tehdy, kdy jsou vyčerpány veškeré možnosti konzervativní terapie a je zapotřebí pacientovi ulevit od bolesti.

Abstract:

The theme of this bachelor thesis deals with the operation of rhisarthrosis, or the osteoarthrosis of root joint of the thumb and nursing care for the patient who has undergone the surgery treatment of rhisarthrosis. The aim of this bachelor thesis is to describe in detail the case of 56 year old patient who suffer from primary osteoarthrosis of the first carpometacarpal joint (M180). In practical part of this thesis is described the immediate preoperative care and also especially postoperative care for a patient, which is focused on management of pain, assessment of the surgical site and safety for patient. Postoperative care is focused on all phases of the nursing process: assessment of health condition, nursing diagnostics, determination of expected results and nursing plan of care for patient, realization of nursing care and evaluation. Data of patient were obtained from primary source – from patient herself, further were used secundary source – medical documentation and medical staff. Techniques wich were used for obtaining data: interview, observation and measuring nursing techniques. Towards consideration a health condition of patient was used the nursing model by Marjory Gordon, which found out functional and unfunctional formulas of health and on their basis were determited nursing diagnosis by NANDA I Taxonomie II 2015-1017. The next phase of nursing process – planning, realization and evaluations are described on nursing diagnoses: Acute pain (00132), Risk of infection (00004) and Impaired sleep mode (00198), which were essential nursing problems for patient in postoperative care. The nursing care for patient was effective, the patient felt well all the time of hospitalization. The patient had enought of information and she was sufficiently educated. Osteoarthrosis of the root joint of the thumb is the largest problem of these days, not only for elderly age categories, but also for younger class. These disease is a degenerative disorder, whose progression can’t be hardly slowed down, for example active joint exersise, or minimum load of joint. The doctor chooses surgical treatment when were exhausted all of options of conversative treatments and it´s neccessary to relieve patient of pain.
