Isolation of mononuclear cells from peripheral blood and their detection by flow cytometry in patients after bariatric surgery treatment.

Abstract:

Mononukleární buňky představují důležitý marker, který lze využít při monitorování a hodnocení celé řady patofyziologických stavů (zánětlivá reakce, kardiovaskulární onemocnění, nádorová onemocnění). Na základě množství mononukleárních buněk a identifikace a kvantifikace některých prozánětlivých faktorů jimi produkovaných, je možné také sledovat projevy a intenzitu zánětlivých reakcí, které provází obezitu a metabolický syndrom. Cílem diplomové práce bylo vypracovat a prakticky zavést metodiku izolace CD14+ mononukleárních buněk z periferní krve od pacientů před a po bariatrické operaci. Tato metodika je založena na využití centrifugace v hustotním gradientu. Vlastní separace CD14+ mononukleárních buněk pak probíhá na koloně s pevnou magnetickou matricí s navázaným antigenem xCD14+ . Detekce a kvantifikace výše uvedené subpopulace mononukleárních buněk byla následně provedena metodou průtokové cytometrie, která umožňuje ověření správnosti izolačního postupu. Touto metodikou bylo zpracováno 10 vzorků periferní krve od pacientů před a po bariatrické operaci typu jejuno-ileálního jejuno-kolického bypassu. Výsledky izolace byly zkontrolovány průtokovou cytometrií. Ve většině případů se podařilo dosáhnout homogenního a po kvantitativní stránce dostatečného výtěžku CD14+ monocytů. Zjistili jsme, že existuje celá řada faktorů, která mohou negativně ovlivnit výsledek izolace, jako např. zpracování vzorku v co nejkratším čase po jeho odběru, transport odebraného vzorku krve do laboratoře, a především správná technika odběru prstence mononukleárních buněk.

Abstract:

Mononuclear cells are an important marker that can be used to monitor and evaluate a variety of pathophysiological conditions (inflammatory reactions, cardiovascular diseases, cancer). Based on the amount of mononuclear cells and the identification and quantification of some of the proinflammatory factors produced by them, it is also possible to monitor the manifestations and intensity of the inflammatory reactions that accompany obesity and metabolic syndrome. The aim of thesis was theoretically process and perform isolation of peripheral blood mononuclear cells from patients before and after bariatric surgery by density gradient centrifugation. Following the obtained cells, lymphocytes that showed antigen expression were separated by immuno-magnetic separation using magnetic beads with bound xCD14 antibody CD14. The success and correctness of the separation was verified by detecting cells by a flow cytometer. By this method, 10 peripheral blood samples were processed from patients before and after bariatric surgery of the type of jejuno-ileal jejuno-colic bypass. The isolation results were checked by flow cytometry. In most cases, a sufficient yield of CD14+ monocytes was achieved in a homogeneous and quantitative manner. We have found that, there are a number of factors that can negatively affect the result of isolation, such as processing the sample as soon as possible after taking it, transport the blood sample to the laboratory, and above all the correct technique of collecting the mononuclear cell ring.
