Methodology of weighted comparison of study data with published studies

Abstract:

This thesis deals with methods for adjusting the structure of own data to published literary sources. Data adjustment is important in terms of comparability, which is one of the key assumptions of bio-statistics, and overall correct interpretation of results. In many cases, the selected comparative studies differ in selection criteria, patient characteristics, the definition of clinical endpoints, and others, and it is impossible to determine whether the differences found are due to treatment alone or input factors. By integrating individual patient data from own study, it is possible to adjust to published descriptive characteristics and avoid bias. However, in many cases, studies are not available as primary data, but only in the form of published summary data, therefore the methods used in this work deal with the use of individual patient data from own study and aggregated statistics from publications. The aim of this thesis is to introduce the reader to the problems of indirect comparison followed by application of selected methods to real data of Czech patients with acute heart failure from the AHEAD register. The practical part deals primarily with algorithms of individual methods with their advantages and limitations and also with the indirect comparison of the AHEAD registry with two international studies TRUE-AHF and RELAX-AHF, mainly in terms of patient mortality. The best weighing results were obtained using the MAIC method, which was further applied when comparing clinical endpoints with published results. In all of the examined endpoints, a decrease in patient mortality was seen after weighing the individual data compared to the original variant. Despite a significant reduction in the mortality and frequency of rehospitalizations, it was not possible to achieve similar results in real world data of patients with acute heart failure, compare to those, obtained in the clinical trials. This is mainly due to the lack of descriptive statistics and the measured variables that could be adjusted, and also the high specificity of the clinical trial population.

Tato diplomová práce se zabývá metodami pro adjustaci struktury vlastních dat na publikované literární zdroje. Adjustace dat je důležitá z hlediska srovnatelnosti, která je jedním z klíčových předpokladů biostatistiky a celkově korektní interpretace výsledků. V mnoha případech se zvolené porovnávané studie liší v selekčních kritériích, charakteristikách pacientů, definici klinických cílových ukazatelů a jiných parametrech, přičemž není možné určit, zda nalezené rozdíly jsou způsobeny léčbou samotnou nebo vstupními faktory. Integrací individuálních dat pacientů z vlastní studie lze adjustovat na publikované popisné charakteristiky a vyhnout se zkreslení. V mnoha případech však nejsou studie dostupné jako primární data, ale pouze v podobě publikovaných sumárních údajů, proto se metody použité v této práci zabývají využitím individuálních dat pacientů z vlastní studie a agregovaných statistik z publikací. Práce má za cíl uvést čtenáře do problematiky nepřímých srovnání s následnou aplikací vybraných postupů na data pacientů s akutním srdečním selháním pocházejících z českého registru AHEAD. Praktická část se zabývá primárně algoritmy jednotlivých metod, jejich výhodami a limitacemi a dále i samotným nepřímým porovnáním registru AHEAD se dvěma mezinárodními studiemi (TRUE-AHF, RELAX-AHF) a to hlavně z pohledu mortality pacientů. Nejlepší výsledky vážení byly dosaženy metodou MAIC, která byla dále aplikována při porovnávání klinických cílových bodů jednotlivých studií. Ve všech zkoumaných ukazatelích bylo po navážení individuálních dat vidět pokles mortality pacientů oproti původní variantě. Navzdory výraznému snížení mortality a frekvencí rehospitalizácií, na obecní populací pacientů s akutním srdečním selháním získané z klinické praxe nebylo možné dosáhnout výsledků podobné těm získaným při vyhodnocování klinické studie. Příčinou jsou zejména chybějící popisné statistiky a měřené proměnné, na které by se mohlo adjustovat a rovněž vysoká specificita populace ze studie.
