Assesment of cut-off levels in functional assays used for diagnostics of hemophagocytic lymphohistiocytosis

Abstract:

Familiární hemofagocytující lymfohistiocytόza (FHL) je závažné autozomálně recesivně dědičné onemocnění. Rychlé stanovení diagnόzy je nezbytné pro výběr správné léčebné strategie. V současné době je známo 5 typů tohoto onemocnění (FHL1-5). Funkční testy, především test cytotoxicity T lymfocytů a test degranulace NK buněk, se používají k přímému potvrzení funkčního defektu. Vhodnost těchto testů se liší podle typu onemocnění. Stanovili jsme referenční meze cytotoxického a degranulačního testu u 10 dospělých kontrol. Výsledky cytotoxického testu u dospělých jsme porovnali s dětmi ve věku 7-12 měsíců a zjistili jsme, že výsledky u obou skupin jsou na stejné úrovni. Nakonec ukazujeme pacienta, u kterého byla diagnostikována FHL5 ještě před klinickým nástupem onemocnění pomocí degranulačního a cytotoxického testu.

Abstract:

Familiar hemophagocytic lymphohistiocytosis (FHL) is an serious autosomal recesive hereditary disease. Fast diagnosis establishment is necessary to make right choice for the treatment strategy. Presently, five types of this disease are known (FHL1-5). Functional tests, mainly T lymphocytes cytotoxic test and natural killer (NK) cells degranulation test, are used to directly confirm an functional defect. These tests are suitable according to the type of the FHL disease. We assessed cut-off levels of cytotoxic and degranulation tests in a group of 10 adults. Results of the cytotoxic test in a group of adults were compared to the children of age 7-12 months and we found that results of both groups are at the same level. Finally, we show a case of a patient, who was diagnosed with FHL5 before a clinical onset of the disease using degranulation and cytotoxic tests.
