Physiotherapy of osteoarthritis of the joints of the lower limbs

Abstract:

Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie při artróze kloubů dolních končetin volbou optimálních fyzioterapeutických postupů, konceptů a metod. Metodika: V této práci byly použity metody rozhovoru k sestavení anamnézy, dále pak pozorování, měření a testování k vytvoření vstupního a výstupního kineziologického rozboru. Pracovala jsem s pacientem s artrózou III. stupně v obou kyčelních kloubech a artrózou II. stupně v obou kolenních kloubech. Po vypracování vstupního kineziologického rozboru byl pro pacienta vytvořen krátkodobý fyzioterapeutický plán na dobu dvou měsíců. Pacient podstoupil během dvou měsíců celkem 10 terapeutických intervencí. K dosažení cílů krátkodobého fyzioterapeutického plánu byly použity techniky měkkých tkání, míčková facilitace, trakce, mobilizační techniky, postizometrická relaxace a strečink. Z jednotlivých metodik a konceptů jsem vybrala metodu proprioceptivní neuromuskulární facilitace, metodu senzomotorické stimulace a cvičení s pomůckami (overball, fyzioball a Thera-Band). Výsledky: Výstupní kineziologický rozbor ukázal zvětšení rozsahu pohybu v pravém kyčelním kloubu do aktivní abdukce ze 40° na 45°. U levého kyčelního kloubu došlo ke zvětšení hybnosti do aktivní abdukce ze 30° na 40°. U pravého kyčelního kloubu se rozsah pohybu do aktivní vnitřní rotace zvětšil o 5° a do zevní rotace se zvětšil o 10°. U levého kyčelního kloubu došlo ke zvýšení aktivní hybnost do zevní i vnitřní rotace o 5°. Podařilo se nám posílit extenzory pravého kyčelního kloubu ze stupně 4- na stupeň 4+, na levé dolní končetině ze stupně 4+ na stupeň 5. Dále byly posíleny abduktory pravého kyčelního kloubu ze stupně 3+ na stupeň 4. Protažitelnost a posunlivost kůže, podkoží a fascií se zlepšila na obou dolních končetinách v oblasti stehen a proximální třetiny lýtek. Trigger pointy byly odstraněny v oblasti m. (musculus) triceps surae oboustranně a v oblasti m. tibialis posterior vlevo. Byla obnovena kloubní vůle pately na obou dolních končetinách a odstranila se blokáda hlavičky fibuly vlevo. Závěr: Jednotlivé terapie napomohli pacientovi ke zvýšení rozsahu pohybu, zvětšení svalové síly, odstranění trigger pointů a uvolnění měkkých tkání.

Abstract:

Aim of the work: Aim of this work is to contribute to solution of problems of physiotherapy of osteoarthritis of the joints of the lower limbs based on choice of optimal physiotherapeutic procedures, concepts and methods. Methodology: In this work were used methods of dialog to draw up case history, then observation, measuring and testing to create input and output kinesiological analysis. I worked with patient with III. grade osteoarthritis in both hip joints and II. grade osteoarthritis in both knee joints. After elaboration of input kinesiological analysis, there was created a short-term physiotherapeutic plan for two months. In two months patient underwent 10 therapeutic interventions. To reach targets from short-term physiotherapeutic plan, there were used techniques of soft tissues, facilitation with soft balls, traction, mobilizations, post-isometric relaxation and stretching. From methods and concepts I chose proprioceptive neuromuscular facilitation, sensorimotor stimulation and exercise with aids (overball, fyzioball and Thera-Band). Results: Output kinesiological analysis showed increase in range of motion in right hip joint into active abduction from 40° to 45°. Range of motion into active abduction in left hip joint increased from 30° to 40°. Active range of motion into internal rotation increased for 5 degrees in the right hip joint and active ROM (range of motion) into external rotation increased for 10 degrees. On the left lower limb there was increase in external and internal rotation for 5 degrees. We achieved to strengthen extensors of right hip joint from stage 4- to stage 4+, on left lower limb from stage 4+ to stage 5. There were strengthened abductors of right hip joint from stage 3+ to stage 4. Extensibility of skin, hypodermis and fascias was improved in area of thighs and proximal third of calves. Trigger points were removed in area of m. triceps surae on both sides and in area of m. tibialis posterior on left side. Also joint play of patella was restored on both lower limbs and blockade of caput fibulae was removed. Conclusion: Individual therapies helped the patient to increase the range of motion, increase muscle strength, remove trigger points and release soft tissues.
