Abstract:

Táto diplomová práca je zameraná na izoláciu baktérie Streptococcus pyogenes od pacientov vo Fakultnej nemocnici v Hradci Králové vo vybranom časovom období, jej identifikáciu pomocou rôznych fenotypových testov a vyšetrenie citlivosti k antibiotikám. V teoretickej časti práce sú mimo iné riešené mechanizmy rezistencie na vybrané druhy antimikrobiálnych látok. Zaujímal nás súbor pacientov, od ktorých boli získané kmene Streptococcus pyogenes, podľa jednotlivých sledovaných rokov či mesiacov, pohlavia, vekových kategórií a druhu odobratého materiálu. Jedným z cieľov bolo vyhodnotenie výskytu rezistencie k makrolidovým antibiotikám fenotypovými metódami. Následne bol u kmeňov Streptococcus pyogenes určený fenotyp MLSB rezistencie. Najvyšší výskyt infekcií spôsobených Streptococcus pyogenes bol zaznamenaný u malých detí a v zimných mesiacoch. V rámci celého sledovaného súboru izolátov bola zistená 9,35% rezistencia na erytromycín a 8,14% rezistencia na klindamycín. Z fenotypov rezistencie bol najčastejšie zastúpený fenotyp M s prevalenciou 64,58%, druhým najčastejším bol inducibilný fenotyp rezistencie (22,22%) a najmenej zastúpený bol konštitutívny fenotyp (13,19%).

Abstract:

This diploma thesis is focused on isolation of Streptococcus pyogenes from patients at the University Hospital in Hradec Kralove in the selected time period, its identification using various phenotypic tests and susceptibility testing to antibiotics. The theoretical part of this thesis deals also with mechanisms of resistance to selected types of antimicrobial agents. We were interested in a set of patients from whom Streptococcus pyogenes strains were obtained, by reference to years, months, sex, age categories and the type of material taken. One of the objectives was to evaluate the incidence of resistance to macrolide antibiotics by phenotypic methods. Subsequently, the MLSB resistance phenotype in Streptococcus pyogenes was determined. The highest incidence of infections caused by Streptococcus pyogenes was seen in young children in the winter months. Resistance to erythromycin was found to be 9,35% and clindamycin resistance was 8,14% within the entire set of isolates investigated. Of the resistance phenotypes, the most common was M phenotype with a prevalence of 64,58%, the second most frequent was the inducible resistance phenotype (22,22%) and the constitutive phenotype was the least frequent (13,19%).
