Multidisciplinary approach for the treatment of chronic pain in a sports patient following a heel fracture: case report

Abstract:

This study presents a clinical case of a 65-year-old woman who after a fall from a ladder suffered a heel fracture and was operated on by open reduction. At the time of the first visit, the patient presented with evident stiffness and pain in the foot. For this reason, during the first sight, the degree of mobility of the deficient limb was assessed both in terms of muscle and joint. The short-term goals are to reduce pain with ice packs and prevent scar adhesions; as regards the medium and long-term objectives, the work was concentrated on improving joint mobility and restoring motor recovery and a correct gait. An improvement of these objectives was not immediate but with the intensification of the sessions and the work that the patient had to carry out at home, it was possible to notice large improvements. It was possible to achieve these goals and these results, thanks to the use of different techniques ranging from closed and open kinetic chain work but, above all, hydrokinetic therapy was fundamental for the patient's recovery. Despite the achievement of the goals for complete recovery after a fracture of this kind it was after about a year.

Abstract:

In questo studio ? esposto un caso clinico di una donna di 65 anni che dopo una caduta da una scala ha subito una frattura calcaneare ed ? stata operata mediante riduzione a cielo aperto. La paziente al momento della prima visita presentava una evidente rigidit? e dolore al piede. Per questo durante la prima vista si ? iniziato a valutare i gradi di mobilit? dell'arto deficitario sia a livello muscolare che a livello articolare. Gli obbiettivi a breve termine ? stato diminuire il dolore mediante impacchi di ghiaccio e prevenire aderenze cicatriziali; per quanto riguarda gli obbiettivi a medio e lungo termine il lavoro lo si ? concentrato nel migliorare la mobilit? articolare e ripristinare il recupero motorio ed un cammino corretto. Un miglioramento di questi obbiettivi non ? stato immediato ma con l'intensificazione delle sedute e del lavoro che la paziente doveva svolgere anche a casa, si sono potuti notare ampi miglioramenti. ? stato possibile il raggiungimento di questi obbiettivi e di questi risultati, grazie all'utilizzo di diverse tecniche che vanno dal lavoro a catena cinetica chiusa e aperta ma, soprattutto ? stato fondamentale la idrokinesiterapia per il recupero della paziente. Nonostante il raggiungimento degli obbiettivi per il recupero completo dopo una frattura di questo genere ? stato dopo circa un anno.
