Comparison of semi-quantitative Curie and SIOPEN score values for neuroblastoma obtained from planar and SPECT/CT scans

Abstract:

U pacientů s neuroblastomem se provádí scintigrafické vyšetření pomocí 123I-MIBG.Součástí výsledku je i semikvantitativní hodnocení. To je standardně prováděno z planárních snímků. Protože u každého pacienta provádíme jak planární, tak SPECT/CT snímek, brzy jsme narazili na otázku, zda není vhodnější provádět semikvantitativní hodnocení ze SPECT/CT snímků. SPECT/CT snímky umožňují díky vyššímu kontrastu detekovat i málo kontrastní léze, zejména pokud jsou v okolí fyziologické kumulace radiofarmaka, a upřesňovat anatomickou lokalizaci lézí. To vše má vliv na počet detekovaných ložisek zvýšené aktivity. Protože v dostupné literatuře se uvádí, že SPECT/CT snímání neovlivňuje výsledné semikvatitativní skóre a tato informace byla v rozporu s našimi zkušenostmi, rozhodl jsem se této otázce věnovat a na našem souboru pacientů provést statistickou analýzu hodnot semikvatitativního hodnocení z planárních a SPECT/CT snímků. Cílem práce je prokázat, zda je či není statisticky významný rozdíl mezi hodnocením z planárních a SPECT/CT snímků.

Abstract:

123I-MIBG scintigraphy is performed in patients with neuroblastoma. The evaluation of the examination includes also semi-quantitative evaluation. This is performed from planar images as a standard. As both planar and SPECT/CT imaging is performed in every patient, we realized that it may be beneficial to perform semi-quantitative evaluation from the SPECT/CT images. Owing to higher contrast, SPECT/CT images enable to detect less contrasting lesions, mainly if these are located near physiological accumulation of radiopharmaceutical, and better describe their anatomical location. This results in higher number of detected lesions of accumulated activity. Current literature says that SPECT/CT imaging does not influence the resulting semi-quantitative scores which is in disagreement with our assumptions. Therefore we decided to deal with this issue and perform a statistical analysis of semi-quantitative scores from planar and SPECT/CT images. The aim of the thesis is to find out whether there is a statistically significant difference in semi-quantitative evaluation between the planar and SPECT/CT images.
