The influence of antero-lateral ligamentous complex injury on postural stability

Abstract:

Hlavním cílem disertační bylo posoudit vliv poranění laterálního ligamentózního komplexu hlezna na posturální stabilitu stoje. Práce je rozdělena na teoretickou a empirickou část s výzkumným šetřením. Teoretická část práce je zaměřena na rešerši literatury s cílem představit problematiku poranění laterálních ligament hlezenního kloubu a posturální stabilitu po tomto typu zranění. V empirické části je metodou plantografie a posturografie zkoumána posturální stabilita bipedálního stoje a stoje na jedné dolní končetině u dvou výzkumných souborů osob. Z výsledků vyplývá: 1)V testu bipedálního stoje jsme u osob po terapii akutní ruptury laterálních vazů hlezna v porovnání jednotlivých měřeních zjistili signifikantní zhoršení posturální stability v šestém týdnu po sejmutí rigidní fixace oproti prvnímu měření v den sejmutí fixace u nezraněné dolní končetiny a v celkové rovnováze. K následně statisticky významnému zlepšení celkové rovnováhy osob po akutním úrazu hlezna došlo v půl roce od sejmutí fixace. U poraněné dolní končetiny jsme v bipedálním nezjistili žádné statisticky významné změny v posturální stabilitě. Velikost plochy konfidenční elipsy a anteroposteriorních posturálních výchylek v sagitální rovině byly ve stoji na jedné dolní končetině statisticky významně vyšší v prvním týdnu po sejmutí fixace ve srovnání s nezraněnou dolní končetinou. 2)V testu bipedálního stoje jsme nezjistili u souboru osob s chronickou mechanickou nestabilitou hlezna statisticky významné změny ve sledovaných parametrech posturální stability mezi zraněnou a nezraněnou dolní končetinou. V testu stoje na jedné dolní končetině jsme u osob s chronickou mechanickou nestabilitou zjistili statisticky významné změny v parametru plochy konfidenční elipsy a anteroposteriorní posturální výchylky. 3)Celková stabilita v bipedálním stoji byla u osob po terapii akutního zranění v šestém týdnu po sejmutí fixace (třetí měsíc po úrazu) statisticky významně horší vež celková stabilita osob s chronickou mechanickou nestabilitou hlezna. Posturální stabilita ve stoji na jedné dolní končetině byla rok po úrazu statisticky významně lepší než posturální stabilita osob s chronickou nestabilitou hlezna.

Abstract:

The objective was to identify changes in postural control during double/single-leg stance in one year after acute lateral ankle sprain in athletes with chronic mechanical ankle instability. The thesis consists of theoretical and empirical part describing scientific trial. The theoretical part is focused on a literature review in order to introduce the issue of injury of the lateral ligaments of the ankle and postural stability for this type of injury. In the empirical part by COP measurement to identify sujects´changes in postural stability during double-leg stance and single-leg stance tests on force plate. The results show: 1)In double-leg stance in patients after therapy of the lateral ankle ligaments rupture (N = 17) was postural stability significantly impaired in the six weeks after removing of rigid fixation. Confidence ellipse of the center-of-pressure area and COP excursion in sagittal plane were in one-leg stance test significantly higher on injured leg during one week after removing plaster than on uninjured leg. 2)We found not significant difference in postural stability in patients with chronic ankle instability (N = 20) in 30s double-leg stance test between injured and unijured leg. Confidence ellipse of the center-of-pressure area and COP excursion in sagittal plane were in one-leg stance test in patients with chronic ankle instability significantly higher on injured leg than on uninjured leg. 3)Balance control in quiet stance was in patients after therapy of the lateral ankle ligaments rupture significantly impaired in the six weeks after removing of rigid fixation than balance control in patients with chronic ankle instability. Postural stability in one-leg stance test was in patients after therapy of the lateral ankle ligaments rupture statistically significantly better in the one year after injury in compare with patients with chronic ankle instability.
