Tool for identifying the potential impact of HLA-KIR interaction on the outcome of hematopoietic stem cell transplantation

Abstract:

Hematopoetická transplantace kmenových buněk (HSCT) je léčebný postup, při kterém se pacientovi s nefunkční kostní dření nebo imunitním systémem podávají zdravé kmenové buňky vhodného dárce, které mají za úkol obnovit produkci krevních buněk. V případě nevhodného výběru dárce mohou nastat po transplantaci komplikace či léčba nebude mít žádoucí efekt. Hlavní roli při výběru vhodného dárce hraje shodnost genů hlavního histokompatibilitního komplexu (HLA) mezi příjemcem a dárcem kmenových buněk. V současnosti se zkoumají další markery, jejichž zohlednění ve výběhu dárce by mohlo zvýšit úspěšnost HSCT. Jedním z takových markerů je interakce mezi HLA pacienta a receptory natural killer buněk (KIR) dárce. Tato interakce může mít buď inhibiční nebo aktivační účinek, což ovlivňuje aloreaktivitu natural killer buňky. Po uvedení do biologické podstaty této problematiky byl v rámci bakalářské práce vyvinut nástroj, který na základě HLA genů pacienta a KIR genů dárce vyhodnocuje jejich vzájemnou interakci pro různé úrovně HLA typizace. Nástroj může být dále využit odborníky v této oblasti pro zjištění vlivu těchto interakcí na výsledek HSCT.

Abstract:

Hematopoietic stem cell transplantation (HSCT) is a procedure in which a patient with dysfunctional bone marrow or immune system is given healthy stem cells from a suitable donor, with the aim of restoring blood cell production. If the donor is not properly matched, post-transplantation complications may arise, or the treatment may not be effective. The primary criterion for selecting a suitable donor is the compatibility of the major histocompatibility complex (HLA) genes between the recipient and the stem cell donor. Currently, additional markers are being studied, whose consideration in donor selection could enhance the success of HSCT. One such marker is the interaction between the patient's HLA and the donor's natural killer cell receptors (KIR). This interaction can have either inhibitory or activating effects, influencing the allo-reactivity of natural killer cells. Following introducing the biological basis of this issue, a tool was developed that evaluates the interaction between the patient's HLA genes and the donor's KIR genes for different levels of HLA typing.This tool can further be utilized by experts in the field to determine the impact of these interactions on the outcome of HSCT.
