The treatment and prognosis of relapsed/refractory chronic lymphocytic leukemia

Abstract:

Chronická lymfocytární leukemie (CLL) je nejčastější leukemií dospělých v západním světě a její klinický průběh je velmi heterogenní. Přibližně polovina pacientů nikdy nedospěje k zahájení terapie, na druhé straně lze definovat úzkou skupinu pacientů s velmi špatnou prognózou bez ohledu na opakované terapie. Základem terapie CLL je v současnosti chemoimunoterapie, jejíž typ volíme s ohledem na biologický stav pacienta, výsledky prognostických markerů a další faktory. V posledních letech, zejména u rizikových pacientů, roste význam nových molekul ovlivňujících signalizaci přes B buněčný receptor (BCR). I přes veškeré pokroky v terapii zůstává CLL, vyjma provedení alogenní transplantace krvetvorných buněk, nevyléčitelným onemocněním. Disertační práce je koncipována jako komentovaný soubor publikovaných prací, které se týkají epidemiologie, prognózy a léčebné strategie u rizikových pacientů s CLL. Získali jsme aktuální epidemiologická data českých CLL pacientů a dosud chybějící informace o managementu CLL v reálné praxi. Prokázali jsme, že prognóza pacientů s relapsem CLL po léčbě FCR (fludarabin, cyklofosfamid, rituximab) zůstává nepříznivá bez ohledu na to, jakým režimem jsou následně léčeni (v čase analýzy nebyly v léčbě CLL k dispozici nové léky jako inhibitory BCR signalizace). Výrazně horší prognózu mají pacienti s relapsem CLL < 24 měsíců. Ověřili jsme, že alemtuzumab je i v současnosti jednou z možností léčby relapsu CLL včetně pacientů s delecí 17p/mutací TP53, a to bez ohledu na typ detekovatelné mutace. Potvrdili jsme, že režim R-Dex (rituximab, dexamethason) je potentní terapií pro pacienty s relabovanou/refrakterní CLL. Významný je jeho efekt u CLL pacientů s imunními cytopeniemi.Vytvořili jsme klinickou studii fáze II, která prokázala slibný efekt O-Dex (ofatumumab, dexamethason) u vysoce rizikových pacientů s předléčenou CLL. Jistým problémem, stejně jako u R-Dex, zůstává jeho vyšší toxicita. Naše výsledky ukazují, že kombinace metod FISH a konvenční cytogenetiky je efektivnější variantou detekce klonální evoluce (CE) u CLL pacientů, přičemž prognosticky nepříznivá CE je asociována s prodělanou terapií. Naše data naznačují, že osmibarevná flowcytometrie je účinnou a méně náročnou metodou detekce minimální reziduální nemoci. Výsledky disertační práce výrazně přispěly k optimalizaci léčby a stanovení prognózy pacientů s relabovanou a refrakterní CLL.

Abstract:

Chronic lymphocytic leukemia (CLL) is the most common leukemia in the Western World and its course is remarkably heterogenous. Approximately half of CLL patients never require treatment; however, there is a group of patients with a very poor prognosis regardless of repeated therapies. Currently, CLL therapy is predominantly based on a combination of a monoclonal antibody and chemotherapy. But a physical fitness, biological prognostic markers and others have a serious impact on final treatment decision. In recent years, new drugs affecting B-cell receptor (BCR) signaling pathway influence a treatment strategy in high-risk CLL patients. Despite a great progress in the last decade, allogeneic stem cell transplantation still represents the only curative approach. The thesis is designed as a collection of published papers regarding CLL epidemiology, prognosis and treatment in relapsed/refractory CLL patients. We have documentated the accurate epidemiologic situation and management of Czech CLL patients in routine clinical practice - data that were missing so far. We have proven that the prognosis of patients who relapsed after FCR (fludarabine, cyclophosphamide, rituximab) remains poor regardless of the subsequent therapy (at the time of analysis new drugs such as BCR signaling inhibitors were not available for CLL treatment). The prognosis is particularly poor in patients with early relapse ˂ 24 months. We have confirmed that alemtuzumab still remains a treatment option for relapsed/refractory CLL patients, including those with deletion 17p/mutation TP53, regardless of the type of mutation. We have proven R-Dex regimen (rituximab, dexamethasone) as a feasible treatment for advanced CLL. Its efficacy is remarkable in patients with immune cytopenias. We had conducted a phase II trial that demonstrated favourable effect of O-Dex regimen (ofatumumab, dexamethasone) in high-risk CLL. Higher toxicity of these 2 regimens remains an important topic. We have suggested a combination of conventional binding analysis and FISH as more effective approach for detecting clonal evolution (CE) in CLL. Poor-prognosis CE was significantly associated with previous CLL treatment. Eight-color flow cytometry appears to be effective and less complicated method for detecting minimal residual disease. In summary, all the mentioned results of this thesis contributed considerably to the treatment optimization and risk stratification in relapsed or refractory CLL patients.
