Validation of nursing diagnostics NANDA International

Abstract:

Úvod: NANDA International je komplexním diagnostickým systémem pro ošetřovatelsky zaměřené obory. Proto jeho taxonomický výbor doporučuje, aby byla před implementací konkrétních ošetřovatelských diagnóz do praxe specifického zdravotnického zařízení provedena jejich důkladná selekce a ověření validity. Hlavní cíle: 1. Realizovat pre a post validační studii vybraných ošetřovatelských diagnóz v kontextu dvou pracovišť intenzivní péče pro dospělé pacienty a 2. Zjistit četnost záznamů ošetřovatelských diagnóz a jejich charakteristických znaků při experimentálním uplatnění inovované ošetřovatelské dokumentace v praxi. Metodika: Obě etapy validace byly provedeny podle postupů modelu Diagnostic Content Validation. Ve všech fázích studie byl zařazen identický soubor 32 diagnóz NANDA International. Ve skupině hodnotitelů první validace bylo 33 expertů, při druhé validaci pak 31. Všichni splňovali minimální kritéria podle Zeleníkové et al., a při realizaci studie působili na zapojených pracovištích jako sestry specialistky. Mezi validacemi hodnotitelé obdrželi inovovanou ošetřovatelskou dokumentaci. S jejím uplatněním provedli ošetřovatelskou diagnostiku u 142 dospělých pacientů, včetně zápisu přítomných charakteristických znaků jednotlivých ošetřovatelských diagnóz. Ke zpracování získaných dat studie byly využity metody popisné statistiky, neparametrický Wilcoxonův (Mann-Whitney) test pro dva výběry a rovněž párový t-test. Výsledky a diskuze: Na podkladě vyjádření hodnotitelů první validace bylo mezi diagnosticky významné zařazeno šest ošetřovatelských diagnóz a při druhé validaci jen tři. Oproti tomu výstupem ošetřovatelské diagnostiky mezi validacemi v praxi, vedené s využitím inovované dokumentace, bylo stejným souborem sester/hodnotitelů u jednoho pacienta zaznamenáno od 2 do 30 různých ošetřovatelských diagnóz NANDA International. Závěr: Studie poukazuje na potenciál uplatnění podpůrných a uživatelsky vstřícných nástrojů při realizaci a dokumentování validní ošetřovatelské diagnostiky v intencích klasifikace NANDA International.

Abstract:

Introduction: NANDA International is a comprehensive diagnostic system for nursing-oriented fields. Therefore, its taxonomic committee recommends that a thorough selection and validation be performed before implementing specific nursing diagnoses in the practice of a specific medical facility. Aims: 1. To implement a pre and post validation study of selected nursing diagnoses in the context of two intensive care units for adult patients and 2. To determine the frequency of records of nursing diagnoses and their characteristics in the experimental application of innovative nursing documentation in practice. Methodology: Both stages of validation were performed according to the procedures of the Diagnostic Content Validation model. An identical set of 32 NANDA International diagnoses was included in all phases of the study. There were 33 experts in the group of evaluators of the first validation and 31 in the second validation. All of them met the minimum criteria according to Zeleníková et al. and during the implementation of the study, they worked at the involved workplaces as specialist nurses. Between validations, evaluators received innovated nursing documentation. With its application, they performed nursing diagnostics in 142 adult patients, including a record of the present characteristics of individual nursing diagnoses. Descriptive statistics methods, a nonparametric Wilcoxon (Mann-Whitney) test for two samples, and also a pairwise method were used to process the obtained study data t-test. Results and discussion: Based on the opinions of the evaluators of the first validation, six nursing diagnoses were included among the diagnostically significant and only three in the second validation. In contrast, the output of nursing diagnostics between validations in practice, conducted using innovative documentation, recorded the same set of nurses/evaluators per patient from 2 to 30 different nursing diagnoses NANDA International. Conclusion: The study points to the potential application of supporting and user-friendly tools in the implementation and documentation of valid nursing diagnostics with the intentions of the NANDA International classification.
