Unicompertmental knee arthroplasty implantation with navigation system - Radiographic survey of short-term and middle-term results

Abstract:

Artróza kolenního kloubu je jednou z nejčastějších příčin omezení mobility populace ve středním a vyšším věku. Poměrně často je postižen pouze jeden kompartment – mediální či laterální. Zavedením miniinvazivní operační techniky a užitím počítačové navigace při implantaci UKA do běžné praxe, vedlo k renesanci implantace UKA. Chceme experimentálně prokázat roli lidského faktoru při převádění počítačem doporučených hodnot do reálné situace. Verifikujeme naší chybovost při implantaci UKA. V klinické části práce sledujeme rentgenologické výsledky s odstupem pěti let od operace. Prokázali jsme, že doporučené hodnoty uložení obou komponent UKA navržené počítačem, jsme schopni převést do reálného stavu bez výrazných chyb. Tzn. že dosažená reálná situace odpovídá virtuálnímu počítačovému modelu. Pouze pokud navržené hodnoty záměrně ovlivňujeme pod vlivem literálních údajů a vlastních zkušeností, stávají se tyto úmyslné chyby statisticky vyznamné. Dále na základě verifikace chybovosti, danou lidským faktorem, jsme prokázali, že jsme schopni dosáhnout ideálního uložení komponent bez statisticky významných chyb. Potvrzujeme signifikantně pozitivní vliv počítačové navigace na korektní uložení komponent UKA v krátkodobém horizontu, charakterizovaný nízkým výskytem neúspěšně implantovaných UKA (36%) po třech měsících od operace. Ve střednědobém horizontu se již výsledky uložení konkrétních UKA v obou skupinách vzdalují od ideálního stavu, nicméně rentgenologické výsledky ve skupině neúspěšně implantovaných UKA jsou signifikantně horší v řádu desetinásobku, ve srovnání s výsledky úspěšně implantovaných UKA.

Abstract:

Arthritis of the knee joint is one of the most frequent causes of the limitation of mobility at the population in the middle and higher ages. Relatively often just one compartment was afflicted – medial or lateral. An introduction of miniinvasive surgical technique and the use of computer navigation and common use of implantation UKA have resulted in the revival of the implantation UKA. We intend to prove experimentally the role of the human factor if values recommended by a computer are transfered into real situation. We verify our incorrectness at the implantation UKA. The clinical part of the work we follow radiographic results with interval five years from surgery. We prove, that we are able to transfer the recommended values of the position of both the components UKA suggested by a computer into reality without distinctive errors. It means, that the achieved real situation complies wtih the virtual computer model. Only our purposeful affecting the suggested values under the influence of literary dates and our own experience makes these intentional errors statistically significant. Furthermore we proved based on the verification of incorrectness resulting from human factor when implanting UKA, that we are able to achieve an ideal position of the components without statistically significant errors. We confirm enormously positive influence of the computer navigation on the correct position of the components UKA in a short-term horizon characterized by a low occurence of the unseccessfully implanted UKA (36%) after three months after surgery. In a middle-term horizon the resultes of the deposition of concrete UKA in both the groups rather depart from the ideal, however Xray results in the group of the unsuccessfully implanted UKA are significant worse almost ten times in comparison with the results of successfully implanted UKA.
