Sexual dysfunction in men after spinal cord injury

Abstract:

Incidence traumatických míšních lézí v Česku je asi 200 případů za rok. Sexuální dysfunkce jsou běžným důsledkem poranění míchy. Cílem práce bylo analyzovat sexuální funkce s ohledem na rozsah postižení a profil kvality života pacientů s míšní lézí. Srovnáním dat s kontrolním souborem jsme získali poznatky, které aspekty kvality života jsou postižením nejvíce limitovány. Hledali jsme faktory korelující s aktivním sexuálním životem, resp. které proměnné predikují zachování sexuálního života po úrazu a mají vliv na jeho kvalitu. Soubor tvořilo primárně 93 mužů po spinálním traumatu. Pro sběr empirických dat byla použita anonymní dotazníkova metoda, validizovaný dotazník kvality života SQUALA, dotazník erektilní funkce IIEF-5 a nestandardizovaný dotazník vlastní konstrukce. Data jsme získali od 43 pacientů, návratnost dotazníků byla 46 %. Byla využita deskriptivní a induktivní statistika s využitím Student T testu (p < 0,05). K testování korelací domén dotazníku Squala byl použitý Personův korelační koeficient. Statistické testování významnosti bylo na hladině významnosti p < 0,01 a p < 0,05. Kontrolní soubor tvořilo 40 respondentů. Transverzální míšní léze predikuje větší rozsah poškození v oblasti senzitivity genitálu, kvality erekce a ejakulace ve srovnání muži s neúplným přerušením míchy. Reflexní erekce je zachována u většiny respondentů bez ohledu na rozsah poškození. U pacientů s transverzální míšní lézi je kvalita koitální erekce závislá na výšce poranění míchy. Čím je léze výše od sakrálního míšního centra erekce, tím je prognóza pro zachování erekce lepší. Rozsah ani výška míšní léze neměly vliv na praktikování sexuálního života, stejně tak ani sexuální touha, mobilita, věk nebo doba od úrazu. Respondenti žijící sexuálním životem mají vyšší vzdělání a jsou více zaměstnáni. Prokázali jsme statisticky signifikantní nižší celkové skóre kvality života mužů po spinálním traumatu ve srovnání s kontrolním souborem, p = 0,048, konkrétně v kategoriích „Zdraví“, p = 0,001; „Fyzická soběstačnost“, p = 0,000; „Děti“, p = 0,003; „Péče o sebe sama“, p = 0,022; „Sexuální život“, p = 0,000 a „ Práce“, p = 0,004. Nebyl prokázán statisticky významný rozdíl v celkové kvalitě života můžů s transverzální a netransverzální míšní lézí. Čím vyšší dosažené vzdělání a věk měl respondent, tím menší důležitost přikládal sexu. Čím delší doba od úrazu, tím vykazovali muži vyšší spokojenost se sexuálním životem bez ohledu na rozsah míšní léze. Z uvedeného vyplývá nutnost individuálního přístupu ke každému nemocnému po míšním traumatu bez ohledu na rozsah postižení.

Abstract:

The incidence of traumatic spinal cord lesions in the Czech Republic is about 200 cases per year. Sexual dysfunction is a common result of spinal cord injury. The aim of our study was to analyze sexual function, considering the extent of the disability and the profile of the quality of life of patients with spinal cord lesions. By comparing the data with the control group, we have gained knowledge which aspects of quality of life are the most impaired and limited. We looked for factors correlated with an active sexual life, respectively variables that predict the preservation of sexual life after injury and affect its quality. The group consisted of 93 men after spinal trauma primarily. The collection of data was made by anonymous questionnaire methods: validated quality of life questionnaire SQUALA, erectile function questionnaire IIEF-5 and non-standardised questionnaire constructed by our own design. Analyses was made by descriptive and inductive statistics using the Student T-test (p <0.05). For the correlation of SQUALAdomain Person correlation coefficient has been used. Statistical significance was tested at a significance level of p <0.01 and p <0.05. The control group consisted of 40 respondents. Data were obtained from 43 patients, response rate was 46%.Spinal cord lesion predicts a greater extent of damage in the genital sensitivity, quality of erection and ejaculation in men compared with incomplete spinal cord transection. Reflective erection is maintained for the majority of respondents, regardless of the extent of the damage. Coital erection quality depends on the level of spinal cord injury for patients with spinal cord lesion. The higher the lesion above the sacral spinal erection centers, the better prognosis for maintaining an erection. Extent and level of spinal cord lesions had an impact on the practice of sexual life, nor does sexual desire, mobility, age or time since injury and trauma. Respondents who declared they have active sexual live have higher level of education and have been emlpoyed more often. We found statistically significant lower overall quality of life score in men after spinal trauma compared with the control group (p = 0.048), specifically in the categories of "Health" (p = 0.001); "Physical self-sufficiency" (p = 0.000); "Children" (p = 0.003); "Self-care" (p = 0.022); "Sexual Life"(p = 0.000) and "Work" (p = 0.004). There was so statistically significant difference in overall quality of life in men with complete and incomplete spinal cord lesions. The higher educational level and age of the respondent, the lower appreciation of the importance of sexual aktivity . The longer the time since the accident (injury and trauma), the higher satisfaction with sexual life, regardless of the extent of spinal cord lesion. We have verified that sexual activity does not depend on the level of spinal cord lesion in selected group of patients. Individual approach to each patient after spinal cord trauma, regardless of the extent of the disability is needed.
