Leukocyte populations in patients with humoral immunodeficiency

Abstract:

Do skupiny humorálních imunodeficiencí je zařazován běžný variabilní imunodeficit (CVID), který představuje heterogenní skupinu onemocnění s neznámou etiopatogenezí. U pacientů jsou nalézány abnormality v ukazatelích specifické imunity, jejichž podíl na poruše tvorby protilátek není objasněn. Méně poznatků je o zapojení nespecifické části imunitního systému, a to především o funkcích neutrofilů a monocytů, ačkoliv nedávno publikované práce ukázují na možnou aktivaci těchto buněk. Cílem práce bylo vyšetření povrchových markerů neutrofilů a monocytů a stanovení přítomnosti neutrofilů s nízkou hustotou (LDNs), které mohou způsobovat supresi aktivace a proliferace T lymfocytů a objasnění vlivu všech těchto typů buněk na produkci IFN-γ a expresi CD25 na T lymfocytárních subpopulacích u pacientů s CVID. Dále byla studována exprese znaků PD-1 na T lymfocytech a PD-L1 na neutrofilech a monocytech jako další možný zdroj suprese T lymfocytů. Naše výsledky nepotvrdily zvýšenou expresi aktivačních znaků na neutrofilech a monocytech ani zvýšení exprese PD-L1 na neutrofilech a monocytech u pacientů s CVID. Zároveň nebylo nalezeno zvýšené procentuální zastoupení populace LDNs. U T lymfocytů pacientů s CVID jsme po aktivaci PBMC detekovali zvýšené hladiny IFN-γ u obou subpopulací T lymfocytů při nižší expresí aktivačního znaku CD25 ve srovnání s kontrolní skupinou. Zjistili jsme rovněž zvýšené hladiny PD-1 na CD4 T lymfocytech. Naměřené výsledky tedy neukazují na zvýšenou aktivitu neutrofilů a monocytů nalezených v periferní krvi pacientů, ale spíše na poruchu regulačních mechanismů mezi oběma zkoumanými skupinami buněk. Součástí práce byla i optimalizace pracovního postupu, konkrétně výběr lyzačního činidla erytrocytů. Nejvhodnějším lyzačním činidlem se ukázal být roztok kyseliny mravenčí, který svou přítomností neovlivňoval sledované aktivační znaky na neutrofilech a monocytech.

Abstract:

Common variable immunodeficiency (CVID) is classified into a group of humoral immunodeficiencies. CVID represents a heterogeneous group of diseases with an unknown etiopathogenesis. Abnormalities in specific indicators of immunity are found in patients with CVID, but their share of the failure of antibody formation is unclear. Little information is known about the involvement of a nonspecific part of the immune system, particularly of the functions of neutrophils and monocytes; although recently published papers suggest a possible activation of these cells. The aim of this diploma thesis is to examine surface markers on neutrophils and monocytes and determine the presence of low density neutrophils (LDNs), which can cause suppression of the activation and proliferation of T lymphocytes and clarify the influence of all these cell types on the production of IFN-γ and CD25 expression on T lymphocyte subpopulations in patients with CVID. Also, expression of markers PD-1 on T cells and PD-L1 on neutrophils and monocytes was investigated as another possible source of suppression of T lymphocytes. Our results confirmed neither a higher expression of activation markers on neutrophils and monocytes, nor an increased expression of PD-L1 on neutrophils and monocytes in patients with CVID. The increased percentage of LDNs was also not found in patients with CVID. After activation of PBMC in patiens with CVID, we detected increased levels of IFN-γ in both subpopulations of T cells and lower expression of activation marker CD25 compared to the control group. We have also found increased levels of PD-1 on CD4 T cells. Measured results did not show increased activity of neutrophils and monocytes found in the peripheral blood of patients, but rather the failure of regulatory mechanisms between both experimental groups of cells. Part of the thesis was the optimization of working procedure, namely the selection of the lysis reagent of red blood cells. The most suitable lysis agent proved to be a solution of formic acid, which did not affect the observed activation markers on neutrophils and monocytes.
