Abstract:

Neuroendokrinní nádory (NETs, neuroendocrine tumours) jsou vzácnou skupinou nádorů, vycházejících z buněk endokrinního systému. Tyto nádory nejčastěji postihují gastroenteropankreatický trakt a plíce. Jejich výskyt je doprovázen různorodými klinickými projevy a jejich diagnostika bývá obtížná. Navzdory jejich pomalému růstu je až u 20 % pacientů již v době diagnostiky zjištěna přítomnost metastáz. Vzhledem k jejich heterogenitě, vzácnému výskytu a náročné diagnostice je důležité věnovat se výzkumu nových terapeutických cílů. Tato bakalářská práce se zabývá genetickými aspekty, příčinami vzniku a biologickou léčbou NETs. V úvodu jsou představeny základní charakteristiky NETs a jejich klasifikace podle morfologických a klinických kritérií. Dále je diskutována role genetiky v patogenezi NETs, s důrazem na klíčové geny spojené s tímto typem nádorů. Další část práce se věnuje biologické léčbě NETs a analýze terapeutických strategií, které cílí na genetické a molekulární charakteristiky těchto nádorů. Jsou diskutovány možnosti terapie, které využívají signální dráhy, jako je PI3K/Akt/mTOR, a inhibitory tyrozinkináz. Cílem této práce je poskytnout komplexní přehled o genetických aspektech, možnostech diagnostiky a léčbě NETs.

Abstract:

Neuroendocrine tumours (NETs) are a rare group of tumours arising from cells of the endocrine system. These tumours most commonly affect the gastroenteropancreatic tract and the lungs. Their occurrence is followed by a variety of clinical symptoms and their diagnosis is often difficult. Despite their slow growth, up to 20% of patients are already diagnosed with metastases at the time of diagnosis. Given their heterogeneity, rarity and challenging diagnosis, it is important to pursue research into new potential therapeutic targets. This bachelor thesis focuses on the genetic aspects, causes and biological treatment of NETs. In the introduction, the basic characteristics of NETs and their classification according to morphological and clinical criteria are presented. The role of genetics in the pathogenesis of NETs is also discussed, with attention to key genes associated with this type of tumors. The next part of the paper focuses on the biological treatment of NETs and analyzes therapeutic strategies that target the genetic and molecular characteristics of these tumors. Therapeutic options that target signaling pathways, such as PI3K/Akt/mTOR, and tyrosine kinase inhibitors are discussed. The aim of this work is to provide a comprehensive overview of the genetic aspects, diagnostic and treatment options for NETs.
