Occupational therapy intervention of school-age children after craniocerebral trauma

Abstract:

Ergoterapeutická intervence u craniocerebrálních postižení je postradatelná tím více pokud se jedná o postižení v dětském věku. Včasnost ergoterapeutické intervence je velice důležitá z důvodu prevence komplikací a sekundárních změn a využití regeneračních schopností nepoškozených struktur. Práce je rozdělena na část teoretickou, speciální a praktickou. V teoretické části jsem se věnovala stručnému přehledu anatomie lebky a mozku, epidemiologii traumat mozku a zaměřila se na craniocerebrální trauma. Ve speciální části práce jsem se orientovala na ergoterapii s využitím přístroje Balance Tutor. V praktické části jsem zpracovala kazuistiku třináctiletého chlapce po craniocerebrálním traumatu. Podrobnou anamnézu pacienta jsem odebrala s využitím lékařské dokumentace, rozhovorem s rodiči a pomocí ergoterapeutických testů, mezi než patří například test na motoriku - MACB2 a test na ADL - Barthelův index. K nácviku chůze jsem využila přístroj Balance Tutor, což je moderní perturbační pohybový pás, který díky svému složení umožňuje nácvik posturální kontroly rovnováhy. Usilovalo se o zlepšení života chlapce po craniocerebrálním traumatu, šlo tedy o zlepšení soběstačnost v denních činnostech a zároveň motorických a kognitivních funkcí pacienta.

Abstract:

Occupational therapy intervention is expendable for craniocerebral inflictions, even more when it occurs in childhood. Timeliness of the occupational therapy is very important for prevention of complications, for secondary changes and for the use of regenerative abilities of undamaged structures. My thesis is divided into several parts- theoretical, special and practical. In the theoretical part, I focuse on brief overview of anatomy of skull and brain and epidemiology of brain traumas with the focus on craniocerebral trauma. Then in the special part, I shifted my attention to occupational therapy with the use of the Balance Tutor device.Practical part of my thesis processes case history of a thirteen years old boy after craniocerebral trauma. I gathered the detailed anamnesis of the patient with the use of: medical records, interview with parents and with the help of occupational therapy tests- such as: MACB2 and ADL test Barth's index. I worked with Balance Tutor device to practice walk. Balance Tutor device is a perturbation treadmill, which thanks to its composition, allows practice of postural control of balance. Goal of this thesis is: Attempting to improve life quality of a boy after craniocerebral trauma, therefore to improve his self-sufficiency in daily tasks and at the same time to improve motor and cognitive functions of the patient.
