The factors affecting well-being of former child oncological pacients

Abstract:

Diplomová práce se zaměřuje na zjištění vztahů mezi identitou a kvalitou života u bývalých dětských onkologických pacientů v kontextu výzkumného projektu QOLOP (The Quality of Life Longitudinal Study in Oncology Paediatric Patients). V rámci celého projektu je zahrnuto několik dílčích prospektivních studií, které zkoumají kvalitu života dětí a dospívajících po léčbě nádorového onemocnění. V této práci si konkrétně klademe za cíl zjistit, jakým způsobem respondenti zakomponovali svou zkušenost s rakovinou do své identity, dále zda rozdílná identita znamená odlišné vnímání kvality života a také zda se výsledky z naší studie shodují s výsledky ze zahraničních studií. V teoretické části představíme sledované proměnné, tedy identitu a kvalitu života, a to hlavně se zaměřením na dospívající populaci a na bývalé onkologické pacienty. Empirická část odhaluje souvislosti mezi identitou a kvalitou života. K dispozici jsme měli data od 96 respondentů. Ze zahraničních studiích jsme využili nejčastěji užívaný dotazník sledující identitu bývalých onkologických pacientů. Respondenti měli na výběr z pěti nabízených možností: pacient (patient); oběť (victim); někdo, kdo byl kdysi nemocný (someone who has had cancer); ten, kdo přežil (survivor); vítěz (conqueror). Analýza rozptylu ukázala statisticky významné vztahy mezi identitou a depresivitou (měřenou pomocí CDI), posttraumatickým stresem (měřeným pomocí CPTS-RI), sebehodnocením (měřeným pomocí Rosenbergovy škály sebehodnocení) a kvalitou života (měřenou pomocí MMQL). Obecně jsme potvrdili vztah mezi identitou a kvalitou života. S nejhůře vnímanou kvalitou života byla spojena identita „pacient“ (identitu „oběti“ si v našem souboru nezvolil žádný respondent). K podobným výsledkům dochází i zahraniční studie. Našly se však i určité odlišnosti. V anglicky mluvících zemích je s nejvyšší kvalitou života spojena identita „survivor“. V našem souboru to byla identifikace s pozicí „vítěz“ a s pozicí „někdo, kdo byl kdysi nemocný“. Pozice „ten, kdo přežil“ (survivor) byla v našem souboru spojena naopak s horší kvalitou života. Ukazuje se, že pozitivní identifikace je spojena s lépe vnímanou kvalitou života a jako v anglicky mluvících zemích, ve kterých se již nepoužívá pojmenování „pacient“ (patient), ale „přeživší“ (survivor), bychom měli vytvořit pro tuto specifickou skupinu nové pojmenování, se kterým by se mohli také pozitivně identifikovat. Z našich výsledků však vyplývá, že pojem přeživší by nebyl nejvhodnějším pojmenováním a měli bychom se zaměřit spíše na pojmenování „vítěz“ (conqueror).

Abstract:

This diploma thesis aims to identify the relationship between identity and quality of life in former pediatric oncology patients, in the context of the QOLOP (Quality of Life Logitudinal Study in Oncology Paedriatic Patients) project. Several studies, focusing on the quality of life in former pediatric oncology patients, are also included in this project. The specific goal of this thesis is to find out how the respondents incorporated their cancer experience into their identities, if the difference in identity implies a difference in the conception of quality of life and also if the results of our study match the findings of the foreign studies. The variables that we studied (the identity and the quality of life) are presented in the theoretical part with the main focus on adolescents and former oncology patients. The empirical part then reveals the connections between identity and quality of life. Data from 95 respondents were used, together with the questionnaire that is used the most in foreign studies focusing on the identity of former oncology patients. The respondents had to choose from five different options: a patient, a victim, someone who has had cancer, a survivor or a conqueror. The analysis of variance showed us a statistically relevant relationships between identity and depresivity (measured by CDI), PTSD (measured by CPTS-RI), self-esteem (measured by the Self-Esteem Scale) and health related quality of life (measured by MMQL). In general, we have confirmed the relationship between the identity and the quality of life. The identity of „patient“ was connected to the worst percieved quality of life, while the identity of „victim“ was not selected by any of our respondents. Foreign studies came to similar conclusions, with several minor differences. The identity of „survivor“ is connected to the highest percieved quality of life in English-speaking countries. On the other hand, between our respondents, the highest quality of life was connected to the identity of „conqueror“ or „someone who has had cancer“. The identity of „survivor“ was connected to the lower quality of life. The positive identification proved to be linked with a higher percieved quality of life. Just like the English-speaking countries, where the term „patient“ was replaced by „survivor“, we should also create a new term for this specific group to identify with. Our results show that the term „survivor“ would not be the most suitable one though and we should focus more on the term „conqueror“.
