Abstract:

Jako téma mé práce jsem si vybrala fyzioterapii u klienta s periferní parézou n. facialis. Toto téma jsem si vybrala z důvodu, že bych chtěla zasvětit čtenáře do problematiky tohoto onemocnění, z důvodu časté záměny s CMP. Cíl práce: Cílem práce je přispět k řešení problematiky fyzioterapie u periferní parézy n. facialis. Metodika: Pacientku jsem vyšetřila 10.12. 2018, kdy jsem provedla vstupní vyšetření. Při vstupním vyšetření jsem odebrala anamnézu, provedla orientační vyšetření mimického svalstva, provedla svalový test dle Jandy (1996) a výbavnost reflexů typických pro danou diagnózu. Na základě vstupního vyšetření jsem stanovila krátkodobý fyzioterapeutický plán, do kterého byly zahrnuty koncepty, metody a terapeutické postupy, které jsem pro svou terapii s pacientkou použila (fyzikální terapie, míčková facilitace dle Jebavé, uvolňovací masáž, metoda sestry Kenny, využití kineziotapu). V průběhu terapie byly provedeny dvě kontrolní vyšetření, díky kterým jsem kontrolovala, zda dochází ke zlepšení stavu pacientky. Po 7 týdnech jsem provedla výstupní vyšetření a na základě tohoto vyšetření jsem stanovila dlouhodobý fyzioterapeutický plán. Výsledky: Při vstupním vyšetření byly stupně svalstva dle svalového testu nulové. Při kontrolních vyšetření byly vidět pokroky, především při zahájení elektrostimulace a ve výstupním vyšetření už byly téměř všechny svaly na stupni 5 kromě m. nasalis, m. levator anguli oris a m. zygomaticus major. Závěr: Při výstupním vyšetření bylo vidět velké zlepšení stavu mimického svalstva na postižené straně obličeje. Z počátku se stav pacientky nezlepšoval, ale největší zlepšení došlo při zařazení elektrostimulace do terapie, kdy během 14 dní došlo téměř k úplnému uzdravení pacientky. Můžeme tedy říct, že terapie a dodržování životosprávy urychlilo čas rekonvalescence.

Abstract:

Introduction: As a topic of my work I chose physiotherapy in a client with peripheral paresis n.facialis. I chose this topic because I would like to introduce the reader to the issue of this disease, because of the frequent confusion with the central stroke. The aim of the work: The aim of this work is to contribute to the solution of physiotherapy in peripheral paresis n. facialis. Methodology: I examined the patient at 10.12. 2018, when I conducted an initial examination. During the initial examination, I took a namatone, performed an indicative examination of the facial muscles, carried out a muscle test according to Janda (1996) and the equipment of reflexes typical of the given diagnosis. Based on the initial examination I set up a short-term physiotherapeutic plan, which included the concepts, methods and therapeutic procedures that I used for my therapy with the patient (physical therapy, Jebava ball facilitation, relaxation massage, Sister Kenny method, kinesiotherapy, active movement) , actively assisted, passive movements). During the therapy, two follow-up examinations were performed to check if the patient's condition was improving. After 7 weeks I performed an exit examination and based on this examination I established a long-term physiotherapy plan. The results: At the initial examination, the muscle grades were zero according to the muscle test. During the control examinations, progress was seen, especially at the start of electrostimulation, and in the output examination almost all the muscles were already at level 5 apart from m.nasalis, m. levator anguli oris and m. zygomaticus major. Conclusion: The final examination showed a great improvement in the condition of mimic muscles on the affected side of the face. Initially, the patient's condition did not improve, but the greatest improvement occurred in the inclusion of electrostimulation in therapy, when within 14 days almost complete recovery of the patient. So, we can say that therapy and living have accelerated recovery time.
