Standardization of nursing care for patients after left-sided heart catheterization.

Abstract:

Úvod: Srdeční katetrizace jsou zavedenou vyšetřovací a terapeutickou metodou u celé řady srdečních onemocnění, ale především u ischemické choroby srdeční. Do finálního výsledku katetrizace, spadá dekanylace sheathu (vodiče) z punktované artérie po výkonu. Právě metoda vytažení sheathu je důležitým aspektem pro eliminaci komplikací, krvácení zmísta punktované artérie a následně rychlou mobilizaci pacienta. Invazivní metody, mezi něž srdeční katetrizace patří, s sebou přináší možná rizika komplikací. Jedná o komplikace spojené právě se získáním cévního přístupu a následné dosažení hemostázy na konci výkonu. Práce je zaměřena na specializovanou ošetřovatelskou péči po levostranných katetrizacích srdce při odstraňování vodiče (sheathu) z tepenného řečiště (arteria radialis) s použitím kompresního zařízení pro hemostázu na tepně. Cíl: Vytvořit ošetřovatelský standard k dekanylaci sheathu (vodiče) z arteria radialis u pacientů po levostranné srdeční katetrizaci (koronarografie, PCI - perkutánní koronární intervence) a testovat vytvořený standard v klinické praxi. Metodika: Neexperimentální longitudinální explanatorní studie s posouzení skutečného účinku zásahu dle ověřované intervence, díky vytvoření standardizovaného postupu pro dekanylaci arteriálního sheathu. Studie byla provedena ve čtyřech fázích. I. fáze - tradiční literární přehled pro analyzování důkazů pro tvorbu postupu ošetřovatelského standardu dle EBP s výběrem důležitých prvků pro koncepci standardu. II. fáze - tvorba vlastního standardu podle metody pro tvorbu standardů a kritérií dle Dynamic Standard Setting System, zaškolení personálu, retrospektívní analýza dokumentace. III. fáze - sledování efektivnosti zavedeného standardu, provedení auditu. Vyhodnocení auditu dle jednotlivých kritérií S, P, V, vyhodnoceno 180 auditů dle námi vytvořené škály (přijatelné úrovně výkonnosti - ALP Acceptable Level of Performance, Capelli at all. 2012). IV. fáze - komparace výsledků. Údaje intervenovaného souboru byly porovnávány v prospektivním sledování (dle námi vytvořeného standardu) s kontrolním souborem pacientů v retrospektivním sledování (bez intervence dle standardu). Soubor byl specifikován dle stanovených kritérií pro potřeby této práce. Výzkumný vzorek tvořilo (N) 354 pacientů po levostranné srdeční katetrizaci z radiálního přístupu. Výsledky a diskuze: Práce měla stanoveno 14 hypotéz, z toho bylo potvrzeno 9 hypotéz, 5 hypotéz bylo zamítnuto. Závěry práce potvrzují, že svýskytem komplikací nesouvisí věk, BMI, dávka Heparinu, velikost sheathu, diagnoza ICHS, podání Kyseliny acetylsalicylové. Naopak výskyt komplikací souvisí s podáním antiagregancií, výskytem bolestivosti, objemem ml vzduchu pro provedení hemostázy v manžetě TR bandu, akutním infarktem myokardu, diabetem a typem katerizace v našem souboru perkutánní koronární intervence (PCI). Za stěžejní výsledek práce považujeme, že díky intervence standardem v prospektivní skupině pacientů, došlo k redukci bolestivosti, naopak ve skupině retrospektivní, úroveň bolesti souvisela zároveň s výskytem hematomu. Závěr: Naše výsledky potvrzují, že správně nastavený standard, může eliminovat výskyt komplikací vnašem případě výkyt hematomu a redukovat bolest při provádění hemostázy na tepně. Ošetřovatelský standard pro dekynyalci sheathu po levostranné srdeční katetrizaci je důležitou součástí výkonu, ze kterého mohou pacienti profitovat.

Abstract:

Introduction: Cardiac catheterization is an established investigative and therapeutic method for a whole scale of cardiac diseases, especially the ischemic cardiac disease. Sheath decannulation from artery after the procedure is the final part of catheterization and it is the method of pulling out the sheath from punctured artery, which is important aspect for complication, bleeding elimination and subsequently fast patients? mobilization. These complications are mostly related to getting the vascular access and reaching the haemostasis after the procedure. The dissertation is focused on specialised nursing care after diagnostic catheterization, when removing the sheath from arterial vessels (arteria radialis) and using compression devices for artery haemostasis. Aim: The aim is to create standardized procedure for sheath decannulation from arteria radialis after a left-heart cardiac catheterization (Angiography, PCI - percutaneous coronary intervention) and to verify the procedure in practice. Methods: Explanatory non-experimental longitudinal study assessing the real effect of establishing the standardized procedure for arterial sheath decannulation. The research was conducted in four phases. Phase I - a review of literature analysing the evidence of a standardized nursing procedure according to the EBP while selecting important elements for the standard conception. Phase II - standard creation according to the methodology for developing standards and criteria DySSSy (Dynamic Standard Setting System), staff training and retrospective analysis of documentation. Phase III - monitoring and auditing the effectiveness of the established standard. Audit evaluation according to various criteria, structure, process, outcome. Evaluation of 180 audits within an established range (ALP Acceptable Level of Performance, Capelli at all. 2012). Phase IV - results comparison. Data has been compared in a prospective monitoring (according to the established standard) with a group of patients retrospectively monitored (without using the standard). The sample, consisted of (N) 354 patients after a left-heart catheterization using the radial approach, has been divided into two groups. Results and discussion: 14 hypotheses were tested, of which 9 have been confirmed and 5 hypotheses have been rejected. The study confirms that the incidence of complications does not relate to the age, BMI, heparin dose, sheath size, diagnosis of ischemic heart dinase or medication acetylsalicylic acid. On the other hand, the incidence of complications associates with the use of antiplatelet drugs, pain incidence, ml volume of air to perform hemostasis cuff of TR band, acute myocardial infarction, diabetes mellitus and percutaneous coronary intervention (PCI). The outcome of the study confirms the reduction of pain in a prospective group as a result of the standard intervention, while the level of pain in retrospective group relates to the incidence of hematoma. Conclusion: The study results prove that the properly set standard can eliminate the incidence of complications, i.e. the incidence of hematoma and the reduction of pain when reaching an artery haemostasis. The Nursing Standard for Sheath Decannulation after left-heart catheterization stays an important part of an intervention, from which patients can benefit.
