Management of bleeding into the upper GIT of peptic etiology

Abstract:

Experimentální ást: Riziko krvácení do horní části zažívacího traktu je u pacienta užívající nesteroidní antirevmatika 3x vyšší. V provedeném experimentu na potkanech bylo prokázáno, že eroze a vředy nevznikají pouze v žaludku nebo duodenu, ale dokonce i v jejunu a ileu. Uvedené změny se objevují již po podání prvních dávek nesteroidních antirevmatik. Část potkanů vykazovala možnost adaptace na podávání tchto substancí. Histologický obraz vředů vzniklých po nesteroidních antirevmaticích se nijak neliší cytoarchitektonikou od vředů klasických. Klinická část: Navzdory progresivnímu rozvoji endoskopických metod je krvácení z peptických vředů gastroduodena stále vážný chirurgický problém. Výběr vhodného a adekvátního operačního výkonu je velmi komplikovaný, je nutné rozhodnou mezi resekcí žaludku a některým méně radikálním výkonem. Rozhodnutí může usnadnit výpočet některých skórovacích systém- Apache II nebo Rockallovo skóre. Pokud jsou hodnoty těchto skóre vysoké je vhodnější provést výkon méně radikální, který u těchto pacient má méně komplikací. U nízkých hodnot daných skórovacích systém mají oba typy operačních postupů podobné výsledky, proto je výhodnější použít výkon resekění, který spolehlivěji ovlivňuje krvácení.

Abstract:

Experimental part: As clinical practice has shown and proven, the risk of bleeding in the upper part of the gastrointestinal tract is three times higher in patients on non-steroidal anti-inflammatory drugs than in the rest of the population. In this experiment (rats), the impairment of the upper GIT in the form of erosions and ulcers spread to the stomach and duodenum, even to the jejunum and ileum. The impairment occurred during the first days of administration. Most rats show a certain degree of possible adaptation to NSA administration. The histological picture and cytoarchitecture of erosions and ulcers induced through the experimentally administered NSAIDs does not differ from conventional peptic ulcers. Clinical part: Despite the progressive development of endoscopic methods, bleeding into the upper GIT still presents a serious surgical problem. The choice of an appropriate surgical procedure is a rather complicated part of the surgical treatment. It is necessary to choose between stomach resection and a less radical procedure. For the purpose of this assessment, it is advised to use the calculation of Apache II score, Rockall scoring system. In case of the high values of Apache II score and of Rockall score, it is more appropriate to choose a less radical, non-resection procedure to stop the bleeding. If the values of Apache II score and Rockall score are low, both resection and more conservative procedures carry the same degree of postoperative morbidity and lethality. In this case it is advisable to apply a resection procedure that presents a more reliable way of bleeding control.
