Postpartum depression in the postnatal period

Abstract:

Cíl: Cílem práce bylo zjistit výskyt emočních poruch v raném postnatálním období včetně rizika vzniku poporodní deprese. Dílčími cíli bylo zjistit, zda je rozdíl ve výsledcích u vzniku poporodní deprese v souvislosti s věkem respondentek, dosaženým vzděláním, rodinným stavem, plánovaným těhotenstvím, paritou respondentek, nedostatkem spánku či se způsobem vedení a ukončení porodu. Metodika: Výzkum byl prováděn v časovém horizontu leden 2015 - březen 2015 na oddělení šestinedělí souběžně ve dvou nemocnicích a to ve Fakultní nemocnici v Ostravě a Vítkovické nemocnici v Ostravě. Výzkumný soubor tvořilo 238 žen po porodu. Vhodnou variantou se stalo dotazníkové šetření, při kterém bylo využito volně dostupného standardizovaného dotazníku, který sleduje pocity žen v předcházejících dnech. Jedná se o Edinburskou škálu postnatální deprese. Výsledky: Bylo zjištěno, že existuje riziko vzniku poporodní deprese v postnatálním období a psychickou stránku matky narušuje především vzdělání, rodinný stav a způsob očekávání příchodu dítěte na svět. Nejvíce ohrožené jsou matky svobodné, středoškolsky vzdělané bez maturity, které navíc dítě nechtějí. Výsledky také ukazují vztah poporodní deprese a poruchy spánku. Závěr: Těhotenství, porod a narození dítěte jsou snad nejvýznamnější životní událostí a největší změna, kterou žena a její partner musí projít. Riziko vzniku poporodní deprese zde existuje a je třeba, aby porodní asistentka detekovala symptomy psychických poruch v postnatálním období, ať už se jedná o baby blues nebo poporodní depresi a zároveň poskytla adekvátní pomoc, aby dopady byly co nejmenší. Také je třeba se zaměřit a vytvořit efektivní screeningový nástroj pro identifikaci rizik psychických poruch již během těhotenství. Abstract:

Objective: The aim of this work is to determine the incidence of emotional disorders in the early postnatal period, including the risk of postpartum depression. The individual objectives were to determine whether the difference in results at the origin of postpartum depression with the age of respondents , educational attainment, marital status, planned pregnancy, parity respondents, lack of sleep, or with the way management and termination of delivery. Methodology: The research was conducted in the timeframe of January 2015 - March 2015 and took place in two confinement departments at two hospitals: University Hospital in Ostrava and Vitkovice Hospital in Ostrava simultaneously. The research sample consisted of 238 women after childbirth. The suitable variant was a survey in which the freely available standardized questionnaire was used. The questionnaire monitors the feelings of women in the past few days within the Edinburgh postnatal depression scale. Results: It was found that there is a risk of postpartum depression in the postnatal period and the mental aspect is especially disrupted by mother´s education, marital status and type of expectation of bringing a child into the world. The most vulnerable are single mothers, college graduates without GCSE, which moreover do not want a child. The results also show the relationship of postpartum depression and sleep disorders. Conclusion: Pregnancy, delivery and child birth are perhaps the most significant life events and the biggest change that the woman and her partner must pass. Due to existing risk of developing postpartum depression, there is a need to detect symptoms of mental disorders in the postnatal period, whether the baby blues or postpartum depression by a midwife and adequately provide assistance that minimize its impacts. There is also need to focus and develop an effective screening tool to identify risks of mental disorders during pregnancy.
