A gene expression perspective on the heterogeneity of KRAS mutant colorectal cancer.

Abstract:

V klinické praxi jsou KRAS mutantní nádory tlustého střeva považovány za homogenní skupinu nádorů. V současnosti se však objevují přímé i nepřímé důkazy o jejich heterogenitě a to nejen v reakci na léčbu, ale i v přežití. Pochopení buněčných procesů zodpovědných za tyto rozdíly je důležité pro výzkum dalších léčebných prostředků. Studium genové exprese v kombinaci s klinickými parametry je využíváno pro odhalení nových biomarkerů a je jednou z technik používaných pro odhalení biologických procesů specifických pro jednotlivé nádory. Hlavním cílem této práce bylo charakterizovat heterogenitu KRAS mutantních nádorů tlustého střeva z hlediska genové exprese a korelace této heterogenity se známými klinickými, histopatologickými a molekulárními markery nádoru kolorekta a přežitím. K dispozici byl unikátní datový soubor z PETACC-3 klinické studie, který obsahuje informace o genové expresi 313 pacientů s KRAS mutací tlustého střeva doplněné o klinické proměnné, molekulární markery a přežití. K charakterizaci genové exprese byla použita metoda analýzy bez učitele. Pomocí shlukovacích metod jsem stanovila 3 skupiny pacientů s KRAS mutantními nádory tlustého střeva. Avšak mezi těmito podtypy a klinickými proměnnými nebyla zjištěna žádná významná asociace a ani se tyto skupiny pacientů nelišily v přežití. Heterogenita nádorů tlustého střeva s KRAS mutací nebyla objasněna jejich studiem na molekulární úrovni. Zdá se, že metoda analýzy bez učitele není schopná identifikovat klinicky a biologicky významné podtypy.

Abstract:

In clinical practice, the KRAS mutant tumours of colon are considered a homogeneous group of tumours. However, currently there is direct and indirect evidence of heterogeneity not only in response to treatment, but also in survival. Understanding the cellular processes responsible for these differences is important for the research of other treatment methods. The study of gene expression in combination with the clinical parameters is used for the detection of new biomarkers and is one of the techniques used to detect specific biological processes for the individual tumours. The main objective of my thesis was to characterize the gene expression heterogeneity of KRAS mutant colorectal cancer in an unsupervised manner and connect these findings with known clinical, histo-pathological and mutational markers of colorectal cancer and prognosis. An unique data set from PETACC-3 clinical study was used for this purpose, which contains information of the gene expression of 313 patients with KRAS mutations in colon accompanied by clinical variables, molecular markers and survival. Unsupervised analysis was used to unveil gene expression heterogeneity. By the means of clustering analysis I defined 3 subtypes of patients with KRAS mutant colorectal tumours. However, there was not determined any significant association between these subtypes and clinical variables and also there were no differences in prognosis. Heterogeneity of colon tumours with KRAS mutation was not clarified by their study on molecular level. It seems that unsupervised analysis is not able to identify clinically and biologically meaningful subtypes.
