Cardiotocography in Midwifery Practise

Abstract:

Tématem bakalářské práce je kardiotokografie v práci porodní asistentky. Práce je ryze teoretické náplně. Kapitoly v ní obsažené zahrnují historické hledisko percepce fetální srdeční činnosti a monitorace stavu plodu, uživatelské rozhraní kardiotokografu, princip snímání na základě změn fetální srdeční činnosti, druhy frekvenčních jevů kardiotachogramu a nepravidelností děložní činnosti zaznamenané na tokogramu, komplexní interpretaci fetálního kardiotokogramu a management antepartální a intrapartální monitorace plodu v práci porodní asistentky. Uvedená tematika je v práci prezentována v korelaci s recentními doporučenými postupy ČGPS ČLS JEP, s majoritním zaměřením se především na klasifikaci FIGO 2015. Nosnou částí práce je kompletní deskripce a integrované hodnocení jevů kardiotokogramu a v rámci uvedených zdrojů, shrnutí dostupných informací a poznatků k nim vztažených. Cílem validního hodnocení kardiotokografických křivek, nejen v práci porodní asistentky, je včasná detekce fetální dekompenzace. Výstupem bakalářské práce jsou kompaktní karty nabízející ucelenou rekapitulaci a explanaci evaluace dlouhodobých, střednědobých a krátkodobých jevů kardiotachogramu.

Abstract:

The subject matter of the bachelor’s thesis is cardiotocography in a midwifery practise. The work is purely theoretical. Included chapters cover the historical development of the perception of fetal heart activity and the monitoring of the fetal condition, the user interface of the cardiotocograph, the principle of detection based on the changes of fetal heart rate, the types of frequency changes of the cardiotachogram, and the irregularities of the uterine activity recorded on the tocogram, the complex interpretation of the fetal cardiotocogram and the management of antepartum and intrapartum fetal monitoring in the midwife's work. The mentioned topic is presented here in correlation with the recently recommended procedures of the Czech Gynecological and Obstetrical Society Czech Medical Association of J.E. Purkyně, with the majority focusing mainly on the FIGO 2015 classification. The central part of thesis contains a complete description and integrated evaluation of cardiotocogram changes, and, within the mentioned sources, a summary of the available information and findings related to them. The aim of a valid evaluation of cardiotocographic curves, not only in the work of a midwife, is the early detection of fetal decompensation. The outcomes of the bachelor's thesis are compact cards offering a comprehensive recapitulation and explanation of the evaluation of long-term, medium-term and short-term cardiotachogram changes.
