Problems of Patient with Amyotrophic Lateral Sclerosis on artificial ventilation of lungs

Abstract:

Úvod: Amyotrofická laterální skleróza (ALS) je degenerativní onemocnění nervové soustavy, které postihuje centrální i periferní motoneurony. Etiopatogeneze není přesně známá, avšak existuje několik teorií, které se snaží příčinu onemocnění vysvětlit. Nejčastějším prvotním příznakem onemocnění je zhoršení funkce horní končetiny, která se projevuje snížením obratnosti i svalové síly. Postupně se projevují i další příznaky onemocnění, jako je např. dysfagie, dysartrie, laryngospasmus či respirační příznaky. Současně dochází k oslabení i ostatních končetin. Onemocnění je v dnešní době neléčitelné, avšak pro zpomalení progrese onemocnění se doporučuje užívat Riluzol. Prognóza onemocnění nebývá příznivá ? většina pacientů umírá do šesti let od počátečních příznaků. Cíle a metodika: Bakalářská páce je rozdělena na část teoretickou a praktickou. Cílem teoretické práce je definovat onemocnění ALS, její etiologii, příznaky, diagnostiku, léčbu a v neposlední řadě paliativní péči, která je v životě pacienta s touto diagnózou nepostradatelná. Hlavním cílem empirické části je charakterizovat potřeby klienta s ALS v domácím prostředí. Dílčím cílem je osvětlit, jak toto onemocnění ovlivňuje nejen jeho život, ale i život celé jeho rodiny. K dosažení cílů empirické části je použita kazuistika pacienta s daným onemocněním. Data jsou sbírána formou nestrukturovaného rozhovoru jak s pacientem, tak s jeho manželkou, studiem ošetřovatelské dokumentace a pozorování. Závěr: Kazuistika přibližuje život klienta s Amyotrofickou laterální sklerózou a jeho rodiny. Klient se účastní projektu Domácí umělé plicní ventilace, kterou zajišťuje ministerstvo zdravotnictví. Díky tomuto projektu je klientovi umožněna léčba v domácím prostředí, která viditelně pozitivně ovlivňuje jeho zdraví - jak fyzické, tak i psychické.

Introduction: Amyotrophic lateral sclerosis (ALS) is a degenerative nervous system disease that affects the central and peripheral motor neurons. Etiopathogenesis is not precisely known, but there are several theories that try to explain the cause of the disease. The most common initial symptom of the disease is worsening upper limb function, which is manifested by reduced muscle strength and dexterity. Gradually, there have been other symptoms such as dysphagia, dysarthria, laryngospasm or respiratory symptoms. At the same time to weaken and other extremities. The disease is curable today, but to slow down the progression of disease are recommended to take Riluzol. The prognosis is not favorable - most patients die within six years of initial symptoms. Aimes and Methodology: Bachelor thesis is divided into theoretical and practical. The aim of theoretical work is to define the disease ALS, the etiology, symptoms, diagnosis, treatment and ultimately palliative care, which is in the life of a patient with this diagnosis is indispensable. The main objective is to characterize the empirical part of the client's needs with ALS at home. A sub-goal is to shed light on how this disease affects not only his life but his whole life and family. To achieve the objectives of the empirical case study is used in a patient with that condition. Data are collected through unstructured interview with both the patient and his wife, the study of nursing documentation and observation. Conclusion: Case describe life of a client with amyotrophic lateral sclerosis and his families. Clients participating in the project home mechanical ventilation provided by the Ministry of Health. This project enabled the client at home treatment that visibly positive effect on his health - both physical and mental.
