Abstract:

Bakalářská práce je věnována tématu "Objemová substituce u zlomenin femuru a zlomenin pánve." Práce je rozdělena na teoretickou a praktickou část. V teoretické části této práce popisuji zlomeniny femuru a zlomeniny pánve jako zdroj život ohrožujícího krvácení, jež je příčinou hemoragického šoku a indikací k masivní tekutinové resuscitaci. Dále se zde zaměřuji na vnitřní prostředí, jeho poruchy, které jsou šokem zapříčiněny a mohou být infuzní terapií, která je součástí léčby šoku, ovlivněny. Ve výzkumné části se zabývám vlivem infuzních roztoků při tekutinové resuscitaci na vnitřní prostředí pacientů s hemoragickým šokem, způsobeným právě zlomeninami femuru, či zlomeninami pánve, v prvních 24 hodinách od úrazu. Dále snahou práce je poukázat na rozdílné dopady u jednotlivých infuzních roztoků a jejich případné výhody a nevýhody ve srovnání s ostatními roztoky.

Abstract:

The Bachelor´s thesis deals with the theme "Volume substitution in femoral fractures and pelvic fractures". It is divided into two parts. The theoretical part describes femoral and pelvic fractures as a source of life-threatening hemorrhage which is the cause of hemorrhagic shock and the indication for administering massive fluid resuscitation. Further this part is focused on disorders of the body´s internal environment that are caused by shock. These disorders can be influenced by infusion therapy that is a standard therapy in treating shock. The practical part deals with effects of infusion solutions on the body´s internal environment during the fluid resuscitation in patients suffering from hemorrhagic shock caused by femoral and pelvic fractures in the first 24 hours after the injury. This part also points out different effects of individual infusion solutions and their advantages or disadvantages in comparison with other solutions.
