Validizace a porovnání dotazníků na sledování kvality života u pacientů s artrózou kolenního a kyčelního kloubu

Validation and comparison of questionnaires for monitoring quality of life in patients with osteoarthritis of knee and hip joints

Abstract:

Úvod. Disertační práce se zabývá problematikou hodnocení kvality života nemocných s osteoartrózou kolenního a kyčelního kloubu. Hlavní cíle jsou zjistit kvalitu života těchto pacientů před a po implantaci totální endoprotézy (TEP) a ověřit v místních podmínkách použitelnost dotazníku, který je k tomuto účelu používán v zahraniční. Úvodní část práce systematicky shrnuje relevantní informace týkající se osteoartrózy, definuje a vymezuje kvalitu života a podává přehled o hodnotících nástrojích, které se používají u pacientů s osteoartrózou. Pozornost je věnována také hodnocení bolesti. Bylo zjištěno, že v ČR není používán žádný specifický dotazník posuzující kvalitu života přímo pacientů s osteoartrózou. Metody. Výzkumná část prezentuje empirická data získaná přímo od pacientů ošetřených na ortopedickém oddělení a v ortopedické ambulanci nemocnice krajského typu. Pacienti byli osloveni před operací a 3 nebo 6 měsíců po ní. Výzkumný vzorek tvořilo 100 respondentů s artrózou kolene a 100 s artrózou kyčle. Využity byly tři hodnotící nástroje současně: kromě dotazníku OsteoArthritis Knee and Hip Quality of Life (OAKHQOL), který je předmětem validizace, také Short Form 36 Health Survey Questionnaire (SF-36) a vybrané kódy International Classification of Functioning, Disability and Health (ICF/MKF). Na posouzení intenzity bolesti byla použita vizuální analogová škála a Dotazník interference bolesti s denními aktivitami. Výsledky. Ve všech hodnotících nástrojích došlo po operaci k podstatným změnám proti stavu před operací. Největší zlepšení bylo v oblastech zachycujících vnímání bolesti a vykonávání fyzických aktivit, ve kterých měli pacienti před operací největší problémy. Vybrané kódy klasifikace ICF/MKF ukázaly na základě hodnocení zdravotníků objektivní zlepšení zdravotního stavu a kvality života respondentů po operaci. Korelace odpovědí získaných pomocí dotazníku OAKHQOL s údaji z dotazníku SF-36 a kódů ICF/MKF klasifikace ukazují na vysokou reliabilitu a validitu v doménách Fyzické aktivity, Psychického zdraví a Bolesti. Naproti tomu, v doménách Sociální aktivity a Sociální podpora je zjištěná reliabilita nízká. Respondenti s artrózou kyčle měli v porovnání s respondenty s artrózou kolene před operací nižší kvalitu života ve většině hodnocených domén a kódů hodnotících nástrojů. Po operaci byla nižší kvalita života u pacientů po TEP kolene. Závěr. Pozitivní efekt operace je patrný už ve velmi krátkém období 3 nebo 6 měsíců po operaci. Využité hodnotící nástroje jsou schopny zaznamenat změny ve zdravotním stavu pacientů a na základě subjektivního hodnocení respondentů změřit kvalitu života. Vzhledem k tomu, že reliabilita dotazníku OAKHQOL je srovnatelně vysoká s reliabilitou dotazníku SF-36, přikláníme se k využití specifického dotazníku OAKHQOL. Dotazník OAKHQOL je možné doporučit k využívání pro zjištění problémů v oblasti fyzických aktivit, psychického zdraví a bolesti, kde měly domény vysokou reliabilitu a validitu. Zbylé dvě domény týkající se sociálních aktivit a sociální podpory vykazují v naší studii nízkou míru reliability a validity a je třeba je ověřit ještě dalšími studiemi.

Abstract:

Background. Thesis deals with the evaluation of a quality of life in patients with an osteoarthritis of the knee and hip joints. The main objectives are to determine the quality of life in these patients before and after implantation of total hip replacement and to check usability of a questionnaire used for this purpose abroad in the local conditions. Relevant osteoarthritis information are systematically summarized in the introductory part, it defines the quality of life and provides an overview of evaluation tools that are used on patients with osteoarthritis. Pain assessment is include too. It has been found that any specific questionnaire assessing quality of life for patients with osteoarthritis is not used in the Czech Republic. Methods. Research section presents empirical data obtained from patients treated at the orthopedic department and the orthopedic clinic of the regional hospital. Patients were approached before an operation and 3 or 6 months after treatment. The research sample was formed from 100 people with knee osteoarthritis and 100 people with hip osteoarthritis. Three evaluation instruments were used simultaneously: the Osteoarthritis Knee and Hip Quality Of Life (OAKHQOL) which is subject of validation, the Short Form 36 Health Survey Questionnaire (SF-36) and selected codes from the International Classification of Functioning, Disability and Health (ICF/MKF). Visual analogue scale and pain interference questionnaire with daily activities were used for assessment of the pain intensity. Results. All three evaluation tools indicated substantial changes between conditions before and after the surgery. Major improvements were in the areas of pain perception and physical activities, in which patients had biggest problems before surgery. Selected codes ICF / MKF showed, based on the evaluation from medical personnel, an objective improvement in health status and life quality of respondents after surgery. A correlation of responses obtained through the questionnaire OAKHQOL with data from the SF-36 questionnaire and codes ICF/MKF classification shows high reliability and validity in the domains of Physical activity, Mental health and Pain. From the other side, the reliability in Social Activity and Social Support domains is low. There was a lower life quality prior to surgery in majority of the evaluated domains and code evaluation tools of hip osteoarthritis respondents compared to respondents with knee arthritis. Quality of life of patients with total knee arthroplasty was lower after the surgery. Conclusions. The positive effect is noticeable already in a very short period of 3 or 6 months after the surgery. Assessment tools are able to reveal changes in the health status of patients and to measure quality of life based on a subjective assessment of the respondents. Given the fact that the reliability of the questionnaire OAKHQOL is comparatively as high as reliability of the SF-36 questionnaire, we incline to use specific questionnaire OAKHQOL. OAKHQOL questionnaire can be recommended for use to detect problems in the physical activity, mental health and pain which were the domains with high reliability and validity. The remaining two domains related to social activities and social support reports low level of reliability and validity in our study and should be verified by other studies.
