Pain of patiens after cardiac surgery

Abstract:

Diplomová práce je zaměřena na problematiku bolesti u pacientů po kardiochirurgické operaci. Cílem praktické části je zjistit, jak pacienti po operaci srdce hodnotí intenzitu své bolesti během šesti pooperačních dnů a odhalit souvislosti mezi jimi udávanou intenzitou bolesti a vybranými determinantami. Dále práce zjišťuje, jak pacienti popisují charakter a intenzitu té nejhorší bolesti, kterou po operaci prožili. Dalším cílem práce je odhalit, které činnosti prováděné v rámci pooperační péče způsobují největší bolest, a jak pacienty po kardiochirurgické operaci bolest ovlivňuje během každodenních aktivit. Na závěr zjišťujeme spokojenost pacientů po operaci srdce se sledováním a tlumením jejich bolesti. Výzkumné šetření bylo provedeno u hospitalizovaných pacientů, kteří podstoupili kardiochirurgickou operaci ve dvou fakultních nemocnicích. Výsledný soubor tvoří celkem 123 pacientů. Ke sběru dat byl použit anonymní dotazník. Výzkumné šetření probíhalo v měsících listopad 2014 až únor 2015. Na základě tohoto šetření bylo zjištěno, že pacienti po kardiochirurgické operaci popisují nejčastěji intenzitu bolesti 0. pooperační den jako mírnou, od 1. do 3. pooperačního dne jako středně silnou a od 4. do 6. dne po operaci jako mírnou. Největší intenzitu bolesti pacienti v průměru udávají 2. pooperační den, kdy nabývá hodnoty 4,63, naopak nejmenší intenzita bolesti je popisována 6. den po operaci s průměrnou hodnotou 1,45. Pomocí SF-MPQ nejvíce respondentů hodnotilo charakter své nejhorší bolesti po operaci jako ?pálivá, palčivá (78,89 %), ?citlivá na dotek? (71,54 %) a ?bodavá? (68,29 %). Průměrná intenzita nejhorší bolesti po operaci byla 5,72. Jako nejbolestivější činnost v pooperačním období uvedli pacienti odkašlávání, převazy a vytahování hrudních drénů. Víc než polovina dotazovaných (51,22 %) své bolesti popisuje tak, že je výrazně neobtěžují a neruší a dá se na ně při činnosti zapomenout. Úroveň podaných informací ohledně způsobu tlumení pooperační bolesti pacienti nejčastěji považují za zcela dostačující (52,85 %). 72,36 % respondentů uvádí, že ošetřovatelský personál hodnotil jejich bolest po operaci pravidelně. U 73,17 % dotazovaných se ošetřovatelský personál zajímal i o ústup bolestí po podání analgetik. Celkově ohodnotilo tlumení pooperačních bolestí jako zcela dostačující 52,85 % pacientů po kardiochirurgické operaci.

Abstract:

The thesis is focused on the issue of pain in patients after cardiac surgery operation. The aim of the practical part is to figure out how the patients after cardiac surgery assesses the intensity of their pain during the six post-operative days and reveal the connection between reported the intensity of the pain and the selected determinants. Further work determines how patients describe the character and intensity of the worst pain experienced after the surgery. Another aim of the work is to reveal which activities carried out in the framework of the postoperative care cause the most pain and how pain affects patients after cardiac surgery operation during everyday activities. In conclusion, we find the satisfaction of patients after cardiac surgery, monitoring and control their pain. Research investigations were carried out for hospitalized patients who underwent cardiac surgery in two university hospitals. The resulting file consists of a total 123 patients. An anonymous questionnaire was used for data collection. Research investigation took place in term November 2014 - February 2015. On the basis of the research investigation, it was found that the patients after cardiac surgery operation 0. post-operative day describe most often the intensity of the pain as mild, from first to third post-operative day as moderately strong and from forth to sixth post-operative day after the surgery, as a slight. Patients describes the greatest intensity of pain second post-operative day with an average value 4.63, and the smallest intensity of the pain is described sixth day after the operation, with an average value of 1.45. Using the SF-MPQ most respondents rated the character of their worst pain after the operation as a "hot, burning (78,89 %), the" sensitive touch"(71,54 %), and" stabbing "(68,29 %). The average intensity of the worst pain after surgery was 5.72. As the most painful activity in the post-operative period, the patients describes during coughs, bandaging and pulling the chest drain. More than half of the respondents (51,22 %) describes their pain so that it significantly not bother them and give them to forget during other activities. The level of information on how to control postoperative pain patients most commonly considered to be entirely sufficient (52,85 %). 72,36 % of the respondents stated that the nursing staff has evaluated their pain after the operation regularly and asked about the retreat of their pain after the administration of analgesics. The nursing staff also reviewed the pain after administration of analgesics by 73,17 % of respondents. Overall, 52,85 % of patients after cardiac surgery operation judged the control of postoperative pain as fully sufficient.
