Conceptual Model of Undergraduate Dental Students' Oral Health Knowledge, Attitudes, and Behaviours

Abstract:

Zdravotníci, včetně lékařů a zubních lékařů, hrají klíčovou roli v kontrole a prevenci nemocí tím, že vedou své pacienty ke zdravému životnímu stylu. Studenti zubního lékařství jsou budoucími vůdci v oblasti ústního zdraví; proto jejich znalosti, postoje a chování související s ústní hygienou (KAB – knowledge, attitudes, and behaviours) mají praktickou hodnotu, protože mohou ovlivnit své vrstevníky, rodiny a místní komunity, aby učinili vhodná rozhodnutí týkající se jejich zdraví úst. V sedmi zemích (4 evropské – Česká republika, Estonsko, Německo a Slovensko a 3 středovýchodní – Libanon, Sýrie a Tunisko) byla provedena řada studií k vyhodnocení orálního zdraví a KAB mezi studenty zubního lékařství s využitím široce používaného nástroje pro tento konkrétní účel, Hiroshima University Dental Behavioral Inventory (HU – DBI). Do následné analýzy bylo zahrnuto celkem 2549 studentů, z toho 1716 (67.3 %) žen, 827 (32.4 %) mužů a šest (0.2 %) genderově různorodých studentů. Nejvíce účastníků bylo ze Sýrie (22 %), následovalo Tunisko (21.7 %), Německo (19.9 %), Slovensko (12.4 %), Libanon (12.4 %), Česká republika (6.7 %) a Estonsko (4.9 %). Při zkoumání kurikula bylo 1456 (57.1 %) studentů preklinických a 1093 (42,9 %) studentů klinického studia. Rovněž 1299 (51 %) studentů neprošlo preventivními stomatologickými kurzy (předpreventivní skupina), zatímco 1250 (49 %) již absolvovalo alespoň jeden preventivní stomatologický kurz (popreventivní skupina). Celkově 491 (19.3 %) studentů uvedlo kouření tabáku alespoň jednou týdně, 408 (18.3 %) uvedlo pití alkoholu alespoň jednou týdně a 2202 (86.4 %) uvedlo problematické užívání internetu. Asi 65.8 % uvedlo návštěvu zubaře alespoň jednou ročně. Kouření tabáku signifikantně (p. < 0.001) častěji uváděli muži (35.9 %) než ženy (11.3 %). Podobně pití alkoholu signifikantně (p. = 0.001) častěji uváděli muži (22.2 %) než ženy (16.5 %). Naopak pravidelné zubní prohlídky byly významně (p. < 0.001) častější u žen (68.2 %) než u mužů (60.9 %). Kromě toho ženy měly vyšší frekvenci problematického používání internetu (87.4 %) než jejich mužští kolegové (84.4 %). Celkové skóre HU-DBI bylo 7.03 ± 1.90. přičemž nejvyšší skóre HU-DBI měla Česká republika (9.34 ± 1.29), následovalo Estonsko (8.09 ± 1.22), Německo (7.67 ± 1.32), Slovensko (7.56 ± 1.73), Libanon (6.67 ± 1.83), Sýrii (6.38 ± 1.83) a Tunisku (6.05 ± 1.83). Studenti. kteří uvedli kouření tabáku alespoň jednou týdně. měli významně nižší skóre HU-DBI (7.14 ± 1.89) ve srovnání s jejich protějšky (6.54 ± 1.88). Podobně studenti, kteří uvedli problematické používání internetu, měli významně nižší skóre HU-DBI (7.33 ± 1.47) ve srovnání s jejich protějšky (6.98 ± 1.92). Studenti, kteří uváděli návštěvy zubního lékaře na pravidelné prohlídky alespoň jednou ročně, měli významně vyšší skóre HU-DBI (7.56 ± 1.71) než jejich kolegové (6.00 ± 1.84). Jak je reprezentováno celkovým skóre HU-DBI. orální zdraví KAB se postupně zvyšovalo od prvního roku (6.45 ± 1.98) do pátého roku (7.47 ± 1.80). Rozdíly ve skóre HU-DBI založené na pohlaví nebyly statisticky významné, což ukazuje na roli pregraduálních zubních osnov při vytlačování/eliminaci rozdílů v oblasti ústního zdraví mezi ženami a muži. Zatímco rozdíly ve skóre HU-DBI podle klinických zkušeností (prekliničtí vs. kliničtí studenti) a načasování kurzů preventivního zubního lékařství (vs. kurzů popreventivního zubního lékařství) byly statisticky významné, regresní modelování odhalilo, že použití preventivních kurzů jako hraniční bod by bylo nejen teoreticky smysluplné, ale také by bylo schopnější předpovídat rozptyl ve skóre HU-DBI. Abstract:

Healthcare professionals, including physicians and dentists, have a crucial role in disease control and prevention by leading their patients to adopt healthy lifestyles. Dental students are the future opinion leaders of oral health; therefore, their oral hygiene-related knowledge, attitudes and behaviours (KAB) are of practical value because they can influence their peers, families, and local communities to make appropriate decisions regarding their oral health. A series of descriptive cross-sectional survey-based studies had been carried out in seven countries; 4 European (Czechia, Estonia, Germany and Slovakia) and 3 Middle-Eastern (Lebanon, Syria, and Tunisia) to evaluate the oral health KAB among dental students utilizing a widely used instrument for this particular purpose, Hiroshima University Dental Behavioural Inventory (HU-DBI). A total of 2549 students were included in the downstream analysis, out of which 1716 (67.3%) were females, 827 (32.4%) were males, and six (0.2%) were gender-diverse students. Syria was the most contributing country (22%), followed by Tunisia (21.7%), Germany (19.9%), Slovakia (12.4%), Lebanon (12.4%), Czechia (6.7%), and Estonia (4.9%). On examining the curricula, 1456 (57.1%) students were pre-clinical, and 1093 (42.9%) were clinical students. Also, 1299 (51%) students were not exposed to preventive dentistry courses (pre-prevention group), while 1250 (49%) were already exposed to at least one preventive dentistry course (post-prevention group). Overall, 491 (19.3%) students reported smoking tobacco at least once a week, 408 (18.3%) reported drinking alcohol at least once a week, and 2202 (86.4%) reported problematic internet use. About 65.8% reported visiting the dentist at least once a year. Tobacco smoking was significantly (Sig. < 0.001) more frequently reported by males (35.9%) than females (11.3%). Similarly, alcohol drinking was significantly (Sig. = 0.001) more frequently reported by males (22.2%) than females (16.5%). Contrarily, regular dental check-ups were significantly (Sig. < 0.001) more common among females (68.2%) than males (60.9%). Moreover, females had a higher frequency of problematic internet use (87.4%) than their male colleagues (84.4%). The overall HU-DBI score was 7.03 ± 1.90, with Czechia having the highest HU-DBI score (9.34 ± 1.29), followed by Estonia (8.09 ± 1.22), Germany (7.67 ± 1.32), Slovakia (7.56 ± 1.73), Lebanon (6.67 ± 1.83), Syria (6.38 ± 1.83), and Tunisia (6.05 ± 1.83). The students who reported tobacco smoking at least once a week had a significantly lower HU-DBI score (7.14 ± 1.89) compared with their counterparts (6.54 ± 1.88). Similarly, the students who reported problematic internet use had a significantly lower HU-DBI score (7.33 ± 1.47) compared with their counterparts (6.98 ± 1.92). The students who reported visiting the dentist for regular check-ups at least once a year had a significantly higher HU-DBI score (7.56 ± 1.71) than their counterparts (6.00 ± 1.84). The overall HU-DBI score shows that oral health KAB gradually increased from the first year (6.45 ± 1.98) to the fifth year (7.47 ± 1.80). The gender-based differences of HU-DBI scores were not statistically significant, thus indicating the role of undergraduate dental curricula in squeezing/eliminating the oral health disparities between females and males. While the differences of HU-DBI scores ac-cording to clinical experience (pre-clinical vs. clinical students) and timing of preventive dentistry courses (pre- vs. post-preventive dentistry courses) had been statistically significant, regression modelling revealed that using preventive courses as a cut-off point would not be only theoretically meaningful but also more capable of predicting variance in HU-DBI score. The country's economic rank as a macro-level determinant of health was weakly yet significantly correlated with students' HU-DBI scores.
