The return to competitive sports activity of a patient with chronic epitrocleitis and impairment of the ulnar nerve: a clinical case

Abstract:

This thesis is about a patient with epitrocleitis. It has been decided to consider a patient who followed a rehabilitative program during the internship period. The patient's name is F. and he is a professional athlete of Brazilian Jiu Jitsu. Precisely for sporting reasons, he was affected by the pathology of epitrocleitis, which was initially ignored, thus making it to worsen, reaching a chronic stage and affecting also the ulnar nerve. The choice of this patient lies in the fact that his pathology was at once orthopedic and neurological, making this case more challenging, but at the same time fascinating and engaging. The whole program will be discussed in the clinical case, so in the second part of the thesis, while in the first half the research aspect will be considered, discussing topics such as anatomy (the skeletal system of the arm, all the muscles involved and ligaments), biomechanics (all movements that can be performed by the joints of the arm and the degrees of movement that each one has) and the epithrocleitis itself (with particular study on the symptoms, causes and prevention of this pathology). Within the clinical case, patient F. was also psychologically evaluated, as sports activity was not seen as a hobby, but as a real well-established lifestyle; therefore the distance from it has led the patient to a suffering not only physical, but above all emotional. Rehabilitation involved a period of 12 weeks, during which the patient underwent several treatments of increasing intensity, according to his needs and responses to them.

Abstract:

La tesi tratta di un paziente affetto da epitrocleite. Si ? deciso di prendere in esame un paziente che ? stato accompagnato in un percorso riabilitativo durante il periodo di tirocinio. Il paziente si chiama F. ed ? un'atleta professionista di Brazilian Jiu Jitsu. Proprio per motivi sportivi ? incorso nella patologia dell'epitrocleite, che ? stata inizialmente ignorata portando cos? ad un aggravarsi della patologia stessa, che ? arrivata in fase cronica ed ? andata ad interessare anche il nervo ulnare. La scelta di questo paziente risiede nel fatto che risulta essere un insieme di una patologia sia ortopedica che neurologica, il che rende il caso pi? impegnativo, ma al tempo stesso affascinante e coinvolgente. Tutto il percorso svolto verr? trattato all'interno del caso clinico, quindi nella seconda parte della tesi, mentre nella prima met? verr? discusso prettamente l'aspetto di ricerca. In tale sezione verranno trattati, quindi, argomenti come l'anatomia (il sistema scheletrico del braccio, tutti i muscoli coinvolti ed i legamenti), la biomeccanica (tutti i movimenti che possono compiere le articolazioni del braccio ed i gradi di movimento che ciascuna possiede) e l'epitrocleite stessa (con particolare approfondimento sui sintomi, le cause e la prevenzione di tale patologia). All'interno del caso clinico, il paziente F. ? stato valutato anche da un punto di vista psicologico, in quanto l'attivit? sportiva non era vista come un hobby, ma come un vero e proprio stile di vita consolidato; pertanto la lontananza da essa ha portato il paziente ad una sofferenza non solo fisica, ma soprattutto emotiva. La riabilitazione ha interessato un periodo di tempo pari a 12 settimane, durante le quali il paziente ? stato sottoposto a diversi trattamenti, di intensit? crescente in base alle sue necessit? e alle risposte agli stessi.
