Physiotherapy for vertebrogenic headaches

Abstract:

Tato bakalářská práce se zabývá problematikou bolestí hlavy na podkladě vertebrogenní etiologie. V teoretické části popisuji funkční anatomii a to především páteř a struktury s ní spojené. Dále popisuji v teoretické části bolest, bolest vertebrogenní a bolesti hlavy. U vertebrogenních bolestí jsem se zaměřila i na etiologii vzniku a obecné znaky, které nalezeneme i u cervikogenních bolestí hlavy. Bolesti hlavy jsem rozdělila v textu na primární a sekundární a zmiňuji se okrajově o migréně, tenzí bolesti hlavy a dalších bolestech hlavy. V kapitole zaměřené přímo na cervikogenní bolesti se zabývám historii pojmu, klinickým obrazem, diagnostikou, etiologií vzniku a léčbou. Možnosti fyzioterapie jsou rozděleny na manuální terapii, pohybovou terapii, fyzikální terapii a další specifické metody. V praktické části jsou uvedeny na začátku výzkumné otázky a cíle a metodika výzkumu bakalářské práce. Další částí je cvičební jednotka s instrukcemi a konkrétní cviky, kde uvádím, u kterého z pacientů bylo cvičení prováděno. Při sestavování cviků a instrukcí, jsem vycházela ze znalostí načerpaných během studia. Cvičební jednotka je pouze ukázkou cviků, které lze využít pro zlepšení stavu pacienta. Tři kazuistiky obsahují vstupní a výstupní vyšetření, popis průběhu terapie a také vliv navrženého cvičení a změny před a po terapii. Na konci bakalářské práce se věnuji diskusi, závěru a uvádím zdroje užité v bakalářské práci. Subjektivně došlo dvou pacientů ze tří ke zlepšení celkového psychického i fyzického stavu. U třetího pacienta došlo ke zdravotním komplikacím, které mu znemožnily pokračovat v terapii. V době výstupního vyšetření měl bolesti spojené s úrazem. Klíčová slova

Abstract:

This bachelor thesis is following up a problematics of headaches originated from vertebrogenic etiology. It will describe functional anatomy in theoretical part of this thesis, primarily a spine and structures associated with it. Furthermore a pain, such as vertebrogenic pain and headaches. It will focus on etiology of vertebrogenic pain and its general symptoms which we can relate to cervikogenic headaches. The headaches will be itemized betweem primary and secundary and it will also partialy mention a migraine, tension headaches and other kinds of headaches. It will focus on history of terms, clinical picture, diagnostics, etiology of origin and therapy in chapter dedicated to cervikogenic pain. Possible physiotherapeutic treatment will be devided on manual therapy, movement therapy, physical therapy and more specific methods. There will be research questions and objectives at the beginning of practical part followed by research methodology of bachelor thesis. Next topic will be exercise along with instructions and substantiation of which exercises has been applied to a specific patient. Compilation of exercises and instructions has been done on base of experience gained throughout the study of physiotherapy. Given exercises are presentment of what can be used to improve the patient's condition. Three case reports will contain entry and exit examinations, description of therapy progress, effect of prescribed exercise and also comparison of patient's condition before and after the treatment. There will be discussion, conclusion and used sources at the end of this thesis. Subjectively said there has been an improvement in two out of three cases in both mental and physical condition of patient. The case with no improvement has been caused by non-related medical complication after injury that prevented further therapy, the patient had pain related to his injury during the exit examination
