Movement biomechanics of surgical lower extremities after an rupture of Achilles tendon

Abstract:

Přetrţení Achillovy šlachy je devastující zranění, které častěji postihuje fyzicky aktivní jedince, kteří se věnují převáţně raketovým sportům jako je tenis a squash nebo také postihuje běţce. Cílem této práce je porovnat kinematiku a kinetiku běhu konkrétního jedince s historií přetrhnutí Achillovy šlachy nejméně jeden rok po operaci se skupinou zdravých jedinců. Odlišuje se tedy kinematika a kinetika pohybu jedince s historií přetrţení Achillovy šlachy při běhu od kinematiky a kinetiky zdravého jedince? Subjektem měření byli rekreační sportovec ve věku 35 let, který před čtyřmi lety podstoupil operaci Achillovy šlachy a šestice probandů bez poranění Achillovy šlachy. Probandi byli párováni na základě podobnosti tělesné výšky a hmotnosti. V této práci se zaměřujeme na výkon v hlezenním kloubu. Probandi podstoupili dvě měření na běţeckém ergometru, na dolní končetiny jim byly umístěny reflexní značky a následné zpracování bylo provedeno v softwaru Qualysis Track Manager a Visual3D. Pro posouzení věcné významnosti jsme vycházeli z indexu velikosti vlivu (ES) podle Cohena. Bylo zjištěno, ţe výstupní výkon v hlezenním kloubu při plantární flexi u patologického běţce byl niţší neţ u zdravých běţců. Můţeme tedy říci, ţe kinetika běhu u jedince s historií přetrhnutí Achillovy šlachy je ovlivněna předchozím zraněním ještě čtyři roky po operaci. Jedinec s přetrţenou Achillovou šlachou je vlivem změněné kinetiky běhu pravděpodobně vystaven většímu riziku zranění při pravidelném běhu neţ nepatologičtí jedinci.

Abstract:

Achilles tendon rupture is a devastating injury that affects more physically active individuals who are devoted to the rocket sports such as tennis and squash and also affects runners. The aim of this study is to compare the kinetics and kinematics of running a particular individual with a history of rupture Achilles tendon at least one year after surgery with a group of healthy subjects. It differs, therefore, kinematics and kinetics of movement of an individual with a history of Achilles tendon rupture while running from the kinematics and kinetics of a healthy individual? The subject measurements were recreational athlete at age 35, who four years ago, underwent surgery Achilles tendon and sis probands without Achilles tendon injury. Probands were paired based on the similarity of body height and weight. In this work we focus on performance at the ankle joint. Probands underwent two measurements of the treadmill ergometer, on their lower limbs were placed reflective signs and subsequent processing was performed in software Qualysis Track Manager and Visual3D. To assess the significance of the substantive we came out of the effect size index (ES) according to Cohen. It was found that the output power at the ankle joint in plantar flexion of pathological runner was lower than in healthy runners. We can therefore say that the kinetics of run at individual with a history of rupture Achilles tendon is influenced by the previous injuries four years after surgery. An individual with break Achilles tendon is influence to change kinetics of run probably expose to greater risk of injury during the regular run than unpathological individuals.
